低分子肝素钠注射液对脓毒症患儿凝血功能的影响

摘　　要：目的:探讨低分子肝素钠注射液对脓毒症患儿凝血功能的影响。方法:对2014年5月—2016年6月河南科技大学第一附属医院收治的脓毒症患儿60例进行回顾性分析,根据治疗方式的不同分为观察组、对照组,每组30例。对照组患儿给予原发病对症治疗,观察组患儿在对照组基础上加用低分子肝素钠注射液。观察两组患儿治疗前后主要凝血功能指标水平及危重程度评分。结果:治疗前,两组患儿各凝血指标水平的差异无统计学意义(P>0.05)。治疗后,两组患儿凝血酶原时间、活化部分凝血活酶时间、凝血酶时间明显短于治疗前,且观察组患儿明显短于对照组;两组患儿血小板计数、纤维蛋白原水平明显高于治疗前,且观察组患儿明显高于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组患儿的危重程度评分为(70.12±3.61)分,明显低于对照组的(78.36±3.56)分,差异有统计学意义(P<0.05)。两组患儿均未出现过敏、皮疹、血肿、皮肤瘀点等不良反应。结论:低分子肝素钠注射液可有效改善脓毒症患儿的凝血功能紊乱及预后情况。OBJECTIVE： To probe into the effect of low molecular weight heparin sodium injection on coagulation function among children with sepsis. METHODS： Retrospective analysis was carried out on 60 children with sepsis admitted into the First Affiliated Hospital of Henan University of Science and Technology from May 2014 to Jun. 2016 were selected and divided into observation group and control group in according to different therapy methods,with 30 cases in each group. The control group was treated with symptomatic treatment of primary disease,while the observation group received low molecular weight heparin sodium injection based on the control group. The main indices of coagulation function and severity rating of two groups before and after treatment were observed. RESULTS： Before treatment,there was no statistical significance in difference of coagulation indices（ P〈0. 05）. After treatment,the prothrombin time,activated partial thromboplastin time and thrombin time of both groups were significantly lower than those of before treatment,and those of the observation group were significantly lower than the control group; the platelet count and fibrinogen levels of both groups were significantly higher than those of before treatment,and those of the observation group were significantly higher than the control group,with statistically significant differences（ P〉0. 05）. After treatment,the severity rating of observation group was（ 70. 12 ± 3. 61） scores,which was significantly lower than that of the control group [（ 78. 36 ± 3. 56） scores],with statistically significant difference（ P〈0. 05）.Adverse drug reactions included allergy, rash, hematoma and petechia were not found in both groups.CONCLUSIONS： The application of low molecular weight heparin sodium injection on children with sepsis can effectively improve the coagulation dysfunction and its prognosis.
