缬沙坦与苯那普利对腹主动脉缩窄兔左室重塑影响的实验研究

摘　　要：目的:探讨缬沙坦与苯那普利对腹主动脉缩窄兔左室重塑的影响.方法:以新西兰雄性大耳白兔60只,随机分为六组,每组10只;按Doering等方法行肾动脉上方腹主动脉部分结扎.苯拉普利组、缬沙坦组和联合组兔于术后2周开始给药,将药品溶于蒸镏水中,以灌胃法喂兔,持续6周;假手术组、手术对照组和正常组则以均等体积的蒸镏水灌胃,标准饲养方法喂养.结果:发现手术对照组(AB-C)颈内动脉压高于假手术组(S)、苯那普利组(AB-B)、缬沙坦组(AB-V)和联合组(AB-BV),P＜0.01;AB-C组右室重量指数(LVM1)亦高于S组、AB-B组、ABV组和AB-BV组,P＜0.05;AB-C组CVF及Ⅰ、Ⅲ型CVF高于S组,P＜0.05;AB-B组CVF及Ⅰ、Ⅲ型CVF明显低于AB-C组,P＜0.01;AB-C组左室含量指数(CVM1)亦高于δ(δ,AB-B组,ABV组和AB-BV组,P＜0.05;AB-V组CVF及Ⅰ、Ⅲ型CVF亦明显低于AB-C组,P＜0.01;AB-BV组与S组相似,P＞0.05.结论:苯那普利和缬沙坦均可显著降低血压、逆转压力负荷增高兔左室肥厚和心肌纤维化,两者无明显差别,但二者半剂量合用时在逆转左室肥厚和心肌纤维化方面效果更明显.Objective: To study the effects of benazepril and valsartan on left ventricular remodeling. Method: 60 male rabbits were randomly divided into six groups including benazepril group (AB- B), valsartan group (AB- V), combined group (AB- BV), operative control group (AB-C), pseudo- operative group (S) and normal group (N), and each had ten animals. Except of S and N, the abdominal aortas of other animals were ligated partly as Doering method. After the operation two weeks, AB - B, AB - V and AB- C had been given the drugs dissolved in water for six weeks, respectively, and the others given water. Result: The internal carotid pressure and left ventricular weight index (LVMI) in AB - B were higher significantly than those of other groups (p < 0. 01 -0. 05), except of the normal group. CVE and CVFs of type I and III in AB - C were higher obviously than those in S (p<0. 05), AB-B (p<0. 01) andAB-V (p<0. 01), respectively. But there was not any significant defference of CVF and CVFs of type I and HI between AB - BV and S. Conclusion: Both benazepril and valsartan can decrease blood pressure, reverse left ventricular hy- pertyophy and myocardial fibrosis in pressure overload model. Little difference of effects has been found between them. When combination of these two drugs at their half a dose, the effects on left ventricular remodeling is better than any monotherapy.
