益气养阴活血通络法治疗早期糖尿病肾病26例临床观察

摘　　要：目的:观察中西医结合疗法治疗气阴两虚络阻型糖尿病肾病早期的临床疗效。方法:选择气阴两虚络阻型早期糖尿病肾病患者47例,随机将其分为治疗组26例及对照组21例。对照组接受降血糖、降血压、抗血小板聚集等西医基础治疗措施,治疗组加服益气养阴活血通络中药汤剂,观察2组糖尿病肾病疗效及中医证候疗效;治疗前后分别观察2组患者血脂、尿白蛋白(UAlb)、尿肌酐(UCr)、尿白蛋白与尿肌酐比值(ACR)。结果:治疗组中医证候疗效和糖尿病肾病疗效均优于对照组,差异有统计学意义。治疗组治疗后UAlb、UCr、ACR、TC、TG水平均较治疗前显著降低,HDL-C水平较治疗前显著升高,且治疗后治疗组UAlb、ACR、TC、TG水平显著低于对照组,HDL-C水平显著高于对照组。结论:在西医常规治疗基础上配合益气养阴活血通络中药可明显提高糖尿病肾病早期疗效。Objective： To observe the clinical outcome of integrated traditional Chinese and western medicine therapy in the treatment of early diabetic nephropathy（ DN） with deficiency of both qi and yin and collateral-obstruction（ DQYCO）. Methods： A total of 47 cases of early DN with DQYCO syndrome were selected and randomly divided into treatment group with 26 cases and control group with 21 cases. The patients in the control group received conventional western medicines such as hypoglycemic agents,antihypertensive agents and platelet aggregation inhibitors; in addition,patients in the treatment group received Chinese medicine prescription for supplementing qi,nourishing yin and dredging collaterals. The clinical outcomes of DN and traditional Chinese medicine（ TCM） syndrome were observed and compared between the two groups. Before and after treatment,the blood lipids,urine albumin（ UAlb）,urine creatinine（ UCr） and UAlb to UCr ratio（ ACR） and observed,respectively. Results： The treatment group had significantly better clinical outcomes of DN and TCM syndrome. After treatment,the treatment group had significant reductions in the levels of UAlb,UCr,ACR,total cholesterol（ TC） and triglyceride（ TG） and a significant increase in the level of high-density lipoprotein cholesterol（ HDL-C）; and the levels of UAlb,ACR,TC and TG in the treatment group were significantly lower and the level of HDL-C was significantly higher than those in the control group. Conclusion： In addition to conventional western medicines,qi-supplementing,yin-nourishing,and collaterals-dredging therapy can obviously improve the clinical outcome of early DN.
