摘 要:为探讨结直肠癌患者PET/CT SUV值和WB-DWI ADC值的相关性,以及不同病理类型结直肠癌SUV值、ADC值的差异,回顾分析31例经病理证实的结直肠癌患者资料.患者均先完成18F-FDG PET/CT显像检查,然后48 h内完成WB-DWI显像检查,分别测量病变SUV(SUVmean、SUVmax)值和ADC(ADCmean和ADCmin)值.采用Spearman相关分析和Wilcoxon秩和检验分析原发病灶SUV值与ADC值之间的相关性,以及腺癌与黏液腺癌SUV值、ADC值差异.结果显示,31例患者中,腺癌25例,黏液腺癌6例.原发病灶SUV值与ADC值之间无相关性:SUVmean与ADCmean无线性相关关系(r=-0.147,P=0.429);SUVmax与ADCmin亦无线性相关关系(r=-0.336,P=0.065).腺癌与黏液腺癌的SUV值、ADC值差异均有统计学意义(SUVmax:P〈0.0001;SUVmean:P〈0.0001;ADCmin:P〈0.01;ADCmean:P〈0.01).腺癌表现为较低ADC值,较高SUV值;黏液腺癌表现为较高ADC值,较低SUV值.结果表明,结直肠癌患者原发病灶PET/CT SUV值和MR-DWI ADC值在黏液腺癌和腺癌中存在差异,这种差异反映了葡萄糖代谢及水分子弥散与肿瘤组织病理类型有一定的关系,两者联合应用可以为分析结直肠癌组织病理类型提供参考,为结直肠癌患者的个体化综合治疗方案提供指导和依据.In order to explore the correlation between standard uptake value(SUV) of positron emission tomography/computer tomography(PET/CT) and apparent diffusion coefficient(ADC) of whole-body diffusion weighted imaging(WB-DWI) in colorectal cancer, and the differences of SUV and ADC values among different pathological types of colorectal cancer, the data of 31 patients with colorectal cancer con- firmed by pathology were analyzed retrospectively. All the patients underwent WB-DWI within 48 hrs after the 18F-FDG PET / CT scan. The lesion SUV (SUVmean, SUVmax) and ADC (ADCmean, ADCmin) were measured respectively. The correlation between SUV and ADC of primary lesion and the differences between SUV and ADC in adenocarcinoma and mucinous adenocarcinoma was analyzed by using the meth- od of Spearman correlation analysis and Wilco3con rank sum test . As results,25 cases were adenocarcino- ma and 6 cases were mucinous adenocarcinoma patients among 31 cases.There was no linear correlation between the SUV and the ADC value of the primary lesion. SUVmean and ADCmean ( r=-0.147, P= 0.429),SUVmax and ADCmin (r=-0.336, P =0.065). There was significant difference in SUV and ADC of adenocarcinoma and mucinous carcinoma (SUVmax: P 〈0.0001; SUVmean. P 〈0.0001 ADC-min, P〈0.01;ADCmean. P〈0.01). Adenocarcinoma showed lower ADC value and higher SUV value; mucinous adenocarcinoma showed higher ADC value and lower SUV value. The rusults show that there are significant difference between SUV and ADC of adenocarcinoma and mucinous carcinoma. The difference reflects there is a certain relationship between glucose metabolism and diffusion of water molecules with tumor pathological type, combined with SUV and ADC can provide a reference for analyzing histopathological types of colorectal cancer and guidance and basis for comprehensive treatment of coloreetal cancer individually.
