改良内括约肌部分切断术治疗慢性肛裂的临床疗效

摘　　要：为探讨改良内括约肌部分切断术治疗慢性肛裂的临床疗效,收集2015年5月至2016年2月大连大学附属新华医院肛肠外科收治的83例慢性肛裂患者资料,其中采用改良内括约肌部分切断术治疗42例（观察组）,采用传统内括约肌切开术治疗41例（对照组）.比较2组患者的总体疗效、创面愈合时间及术后并发症.结果显示,观察组痊愈37例,好转3例,无效2例,总有效率为95.2％（40/42）;对照组痊愈25例,好转7例,无效9例,总有效率为78.0％（32/41）.观察组总有效率明显高于对照组,P〈0.05.观察组创面愈合时间[（9.6＆#177;0.7） d]明显短于对照组[（18.3＆#177;0.6） d],P〈0.05.观察组术后肛门疼痛、肛缘水肿情况明显轻于对照组,P〈0.05.结果表明,改良内括约肌部分切断术治疗慢性肛裂疗效确定,创面愈合快,并发症轻,值得临床推广应用.In order to explore the clinical effect of improved internal sphincter partial incision in the treatment of chronic anal fissure, the data of 83 patients with chronic anal fissure admitted and treated in authors＇ department,Xinhua Hospital affiliated to Dalian University from May 2015 to February 2016 were collected,42 cases （as observation group） were treated with improved internal sphincter partial incision, and rest 41 cases （as control group） with traditional internal sphincterotomy.The overall efficacy,wound- surface healing time and postoperative complications of the two groups were compared.As results, in obser- vation group 37 cases recovered,3 cases got improved, 2 cases were of non-response,and total effective rate was 95.2% （40/42） meanwhile in control group,25 cases,7 eases,9 cases,and 78.0% （32/41） respectively. The total effective rate of observation group was significantly higher than that of control group（ P〈0.05）. The wound-surface healing time in observation group [（9.6±0.7） d] was significantly shorter than that in control group ^（18.3±0.6） d], P 〈0.05. The anal pain and anal edge edema after surgery in observation group were significantly lighter than that in control group（ P〈0.05）. The results show that the improved internal sphincter partial incision for chronic anal fissure has certain efficacy and fast wound-surface healing, mild complications, worthy of clinical popularization and application.
