1例犬深层脓皮症并发医源性肾上腺皮质机能亢进的诊治

摘　　要：犬深部脓皮病是表面或毛囊的细菌感染破坏毛囊的结构造成疖病和蜂窝织炎,由于滥用抗生素或机体免疫力下降所致。通过临床检查、皮肤镜检、细菌培养和药敏试验、皮肤组织学检查以及地塞米松抑制试验和促肾上腺皮质激素(ACTH)刺激试验结合的方法,对1例犬久治不愈的严重皮肤病病例进行诊断,结果确诊为深层脓皮症并发医源性肾上腺皮质机能亢进,导致皮肤严重钙化。根据药敏试验,用恩诺沙星按照5mg/kg,每天1次,口服;维克派愈妥(主要成分为3%过氧化苯甲酰)每3 d药浴1次,配合口服不饱和脂肪酸,并停用原来所有药物,患犬恢复良好。Canine deep pyodermia is commonly caused by bacterial infection that damages hair follicles structure and thus causes furunculo-sis and cellulitis, and then the misuse of the antibiotics or the decreased immune response aggravate this situation. In this study, clinical anddermatoscope examination, bacterial culture and susceptibility tests, skin histological examination among with dexamethasone suppresion testand adrenocorticotropic hormone stimulation test were performed to diagnose one case of canine severe skin disease. Results showed that thecanine patient suffered from deep bacterial pyoderma, simultaneously complicated by iatrogenic hypercorticism, which resulted in severe skincalcification. According to the drug-susceptibility test data, enrofloxacin was used at a dosage of 5mg/kg once a day orally. Virbac paxcutol（the main active component is 3% benzoyl peroxide） medicated bathing was prescribed 3 times a day in combination with orally administra-ted unsaturated fatty acids. And all previous pharmaceuticals were stopped. After the treatment, the patient recovered successfully.
