郑州市2015年轻、重症手足口病患儿的临床体征、实验室指标动态分析

摘　　要：目的:探讨轻、重症手足口病患儿实验室指标随时间的动态变化及重症化结局的影响因素。方法:采用问卷调查收集187例手足口病患儿的基本情况和实验室指标,将患儿分为轻症组和重症组,实验室指标分为不同发病时间组,分析轻、重症患儿的实验室指标变化情况及重症化结局影响因素。结果:轻、重症手足口病患儿的中性细胞比率、淋巴细胞比率、Ig M、降钙素原均随时间动态变化(P<0.05);中性粒细胞比率(OR=1.198,P=0.001)、淋巴细胞比率(OR=1.194,P=0.002)、白细胞个数(OR=0.916,P=0.024)、血清钠离子(OR=0.690,P<0.001)是手足口病患儿重症化结局的影响或预警实验室指标。结论:手足口病患儿病程中免疫水平处于动态变化,且影响重症化结局。Aim： To study dynamic changes of laboratory indicators of children with mild and severe hand,foot and mouth disease（ HFMD） and influential factors of severe outcome. Methods： General information and laboratory indicators were collected from 187 HFMD patients by questionnaire. Patients were allocated into mild and severe groups and then further allocated into 5 groups according to the onset time. Then the changes of laboratory indicators of the patients and influential factors of severe outcome were analyzed. Results： Neutrophils and lymphocyte ratio,Ig M and procalcitonin changed dynamically in HFMD patients（ P〈0. 05）; neutrophils（ OR = 1. 198,P = 0. 001）,lymphocyte ratio（ OR = 1. 194,P =0. 002）,leukocyte count（ OR = 0. 916,P = 0. 024）,serum sodium（ OR = 0. 690,P〈0. 001） might affect or reflect the severity of HFMD. Conclusion： The immune level changes dynamically in children with HFMD,which might affect the severe outcome of HFMD.
