肺心病心衰伴室性心律失常患者的HRT与预后

摘　　要：目的探讨心率减速力值对肺心病右心衰竭恶性室性心律失常患者的预后分析。方法应用24 h动态心电图记录65例肺心病右心衰竭恶性室性心律失常患者组(观察组)接受电除颤器、电复律及相关药物救治措施和65例健康者组(对照组)的心率减速力值预后进行对比分析。结果 65例肺心病右心衰竭恶性室性心律失常患者组的病死率:DC值≥4.5ms 24h内为1.53%,2周内为3.07%,1年内为6.15%;DC值≤2.5ms 24h内为7.69%,2周内为10.76%,1年内为15.38%,各值比较p<0.05;65例对照组心率减速力各值中无死亡病例。结论肺心病右心衰恶性室性心律失常的存活率较低,病死率很高,对该类患者应立即采取电除颤器、电复律及相关药物救治措施,改善预后,降低死亡率。心率减速力值测定能定量、单独分析和测定迷走神经作用的强度,对肺心病心力衰竭高危人群筛选与预警具有较强的实用价值。Objective To investigate prognostic of deceleration capacity of heart rate for pulmonary heart disease right heart failure patients and malignant ventricular arrhythmia ventricular arrhythmia. Methods The deceleration capacity of rate were recorded with 24-hour dynamic electrotrocardiongrm the sixteen-five patients with pulmonary heart disease right heart failure（observed group）electroconversion, and medicine cure must be used at once and the sixteen-five patients with healthy people（compared people group）prognostic analysis done. Results In 24-hour cure succeed ratio in and mortality sixteen-five patients pulmonary heart disease right heart failure with ventricular arrhythmia group： DC≥4.5 ms in 24-hour mortality was 1.53%, in two weeks mortality was 3.07%, in one year mortality was 6.15%; DC≤2.5 ms in 24-hour mortalitywas 7.69%, in two weeks mortality was 10.76%, and in one year mortality was 15.38%, comparison of the forme every value, p〈0.05. and sixteen-five patients compared people group every value not have mortality. Conclusions The survival of malignant ventricular arrhythmia with pulmonary heart disease right heart failure patients was lower and mortality was very high. To the kind patient, electricl defibrillator, electroconversion,and medicine cure must be used at once to improve pyognosis and lower mortality. Deceleration capacity of heart rate value of determined can fixed, alone analysis of and determined vagus nerve effect of strength to pulmonary heart disease right heart failure patients and malignant ventricular arrhythmia ventricular arrhythmia patients were sudden death high-danger crowd screening and pre warning have very strong of useful value.
