摘  要:目的探讨色素上皮衍生因子(pigment epithelium-derived factor,PEDF)在植入型凶险性前置胎盘中的表达及其对脐静脉内皮细胞(human umbilical vein endothelial cells,HUVEC)迁移、侵袭能力的影响。方法选择2014年7月~2015年7月福建省立医院剖宫产分娩的植入型凶险性前置胎盘(PA)组14例,前置胎盘(PP)组16例。采用免疫组化法检测两组胎盘的PEDF和VEGF的定位表达及微血管密度(microvascular vascular density,MVD);用RT-PCR及Western blot检测胎盘的PEDF和VEGF的mRNA和蛋白质表达;用ELISA检测外周血清中PEDF和VEGF水平。培养脐静脉内皮细胞,设阴性对照组和不同浓度的PDEF干预组,用平板划痕和Transwell侵袭实验检测HUVEC细胞的迁移和侵袭能力,用ELISA检测培养上清液中VEGF水平。结果 PA组与PP组相比,胎盘PEDF的mRNA和蛋白表达明显降低而VEGF则明显升高,差异均有统计学意义(均P<0.05)。胎盘PEDF表达与胎盘VEGF、MVD及胎盘植入深度呈负相关关系(均P<0.05),与外周血清PEDF水平呈正相关关系(P<0.05)。各PEDF干预组的迁移和侵袭细胞数显著低于阴性对照组(均P<0.05)。随干预浓度增加,HUVEC细胞的迁移和侵袭能力降低(均P<0.05)。结论 PEDF可以反映植入型凶险性前置胎盘的异常血管密度和植入程度,抑制血管内皮细胞的迁移和侵袭能力,可作为影像学检查的重要补充诊断方法和潜在的治疗切入点。Objective To explore the expression of pigment epithelium-derived factor(PEDF)in placenta previa accreta and its influence on the migration and invasion of vascular endothelial cells.Methods Totally 14 cases of pernicious placenta previa accrete(PA)and 16 cases of placenta previa(PP)were collected between July 2014 and July 2015 from Fujian Provincial Hosptal.The location and expression of PEDF/VEGF and the microvascular vascular density(MCV)were detected in the placentas by immumohistochemical staining.The mRNA and protein expression of PEDF/VEGF was measured by RT-PCR and Western blotting,respectively.The levels of PEDF/VEGF in peripheral blood were detected by ELISA.HUVECs were primarily cultured and devided into control group and PDEF intervention groups.The cell migration and invasion were measured by wound healing assay and Transwell chamber assay.The VEGF level of the supernatant was tested by ELISA.Results The mRNA and protein expression of PEDF in the placentas was significantly decreased and that of VEGF was markedly increased in PA group compared with PP group(P0.05).PEDF expression in the placentas was negatively correlated with MVD and the depth of invasion,and positively with the serum level of PEDF(P0.05).The cell number of migration and invasion was significantly decreased in PEDF intervention groups compared with control group(P0.05).The migration and invasion were gradually reduced with the increase of PEDF concentration(P0.05).Conclusion PEDF is indicative of the density of abnormal vessels in placenta previa accrete and the invasion depth.It may inhibit the migration and invasion of vascular endothelial cells.Therefore,PEDF detection may become an important diagnostic method supplementary to the imaging examinations and PEDF may become apotential therapeutic agent for placenta previa accreta.
