摘 要:目的分析高频超声、X线、CT三种影像学检查方法诊断痛风性关节炎的应用价值。方法选取84例痛风性关节炎患者,分别进行高频超声、X线、CT检测,比较三种影像学检查结果。结果 HFUS、CT、X线检查痛风石、关节积液、组织肿胀检出率依次下降,组间差异有统计学意义(P<0.05),仅HFUS检测出41例患者出现滑膜增厚,X线、CT均无检出;HFUS、CT、X线确诊率依次为89.2%、39.3%、57.1%,HFUS灵敏度最高,X线最低,组间差异有统计学意义(P<0.05)。结论高频超声对痛风性关节炎具有较高的灵敏度,诊断符合率显著高于X线、CT,X线确诊率最低,可用于中晚期诊断。Objective To analyze the application value of high-frequency ultrasound,X-ray and CT in the diagnosis of gouty arthritis. Methods A total of 84 patients with gouty arthritis in our hospital were taken as the clinical research objects. High-frequency ultrasound,X-ray and CT examination were performed respectively,and their results were compared and analyzed. Results The detection rates of HFUS,CT,X-ray examination in detecting tophi,joint effusion and tissue swelling were decreased in turn,and their differences were statistically significant( P〈0. 05); Synovial thickening which could not detected by CT and X-ray was detected in 41 patients by HFUS; The diagnosis rate of HFUS was 89. 2% which was significantly higher than that of CT 39. 3% and X-ray 57. 1%,and their differences were statistically significant( P〈0. 05). Conclusion High-frequency ultrasound,which diagnostic coincidence rate is obviously higher than CT and X-ray,has the highest sensitivity in diagnosis of gouty arthritis. Therefore,it can be used in the diagnosis of middle and advanced gouty arthritis.
