尿肝素结合蛋白检测在尿路感染诊断中的价值

摘　　要：目的探讨尿液肝素结合蛋白水平检测在尿路感染诊断中的价值。方法收集75例患者为观察对象,另收集健康者30例为健康对照组,均进行中段尿培养检查,检测尿液肝素结合蛋白浓度、尿常规分析、尿有形成分测定;应用ROC曲线计算尿液肝素结合蛋白(U-HBP)、白细胞酯酶(LEU)、亚硝酸盐(NIT)、细菌(BACT)计数及尿液白细胞(U-WBC)计数的曲线下面积(AUC);计算尿HBP、LEU、NIT、BACT及WBC检测对首诊尿路感染的敏感度、特异度、阳性预测值、阴性预测值及准确度。结果 UTI组U-HBP的检测结果与其他3组比较,差异均有统计学意义(P<0.05);U-HBP、LEU、NIT、BACT、U-WBC诊断尿路感染的AUC分别为0.882、0.650、0.712、0.816和0.758,U-HBP诊断尿路感染的敏感度和特异度最高,分别为93.5%和89.9%。结论 U-HBP能区分开UTI组和可疑UTI组;尿液肝素结合蛋白检测具备良好的敏感性、特异性,检测也相对方便、快捷,可作为诊断尿路感染的辅助指标。Objective To evaluate the significance of urine heparin-binding protein（ HBP） in the diagnosis of urinary tract infection（ UTI）. Methods 75 patients were collected as the observation objects,and 30 healthy subjects were collected in the healthy control group. Urine samples of all patients and 30 healthy subjects were cultured for the detection of urine heparin binding protein concentration,analysis of urine routine,determination of urine composition; ROC curve was used to calculate the area under curve（ AUC） of urine heparin binding protein（ U-HBP）,leukocyte（ LEU）,nitrate（ NIT）,bacterium（ BACT）and urine white blood cell（ U-WBC）; U-HBP,LEU,NIT,BACT and U-WBC were used to detect the sensitivity,specificity,positive predictive value,negative predictive value and accuracy of the first diagnosis of urinary tract infection.Results There was statistical significance on the difference of the results of U-HBP in UTI group compared with the other three groups（ P〈0. 05）; the AUC of U-HBP,LEU,NIT,BACT and U-WBC in the diagnosis of urinary tract infection were 0. 882,0. 650,0. 712,0. 816 and 0. 758,respectievly. Heparin-binding protein had the highest sensitivity and specificity（ 93. 5% and 89. 9%） in diagnosing urinary tract infection. Conclusion U-HBP can distinguish the UTI group from suspicious UTI group; the detection of urine heparin-binding protein has good sensitivity and specificity,and the detection is also relatively convenient and fast,which can be used as an assistant index for the diagnosis of urinary tract infection.
