摘 要:目的了解该地区血流感染病原菌的分布及耐药特点。方法回顾性分析2014年1月-2016年5月血培养标本中分离病原菌种类及其药敏检测结果。结果病原菌血培养阳性率为16.2%,革兰阴性杆菌占71.8%,其中大肠埃希菌143株占23.0%,肺炎克雷伯菌121株占19.5%,马耳他布鲁菌77株占12.4%。革兰阳性菌152株占24.5%,真菌23株占3.7%。大肠埃希菌和肺炎克雷伯菌产ESBL菌株的检出率分别为44.1%和43.3%。大肠埃希菌、肺炎克雷伯菌、鲍曼不动杆菌对碳青霉烯的耐药率分别为0.7%、6.6%和61.9%。阿米卡星对分离的革兰阴性菌表现出高度的抗菌活性。未发现对万古霉素和利奈唑胺耐药的阳性球菌。结论佳木斯地区血流感染以革兰阴性杆菌为主,大肠杆菌和肺炎克雷伯菌是血流感染的首要致病菌,且该地区马耳他布鲁菌感染应引起重视。阿米卡星对分离菌保持着较好的敏感性。Objective To describe the prevalence and antimicrobial resistance of pathogenic bacteria causing bloodstream infections. Methods Pathogen types and drug sensitivity test results of blood specimens collected from patients during January 2014-May 2016 were retrospectively reviewed. Results The positive rate of blood cultures was 16. 2%,negative Gram bacilli accounted for 71. 8%,of which Escherichia coli,Klebsiella pneumonia and Brucella accounted for 23. 0%,19. 5% and 12. 4%,respectively. The prevalence of positive gram cocci and Fungi were 24. 5% and 3. 7%,respectively. The detection rate of ESBL strains produced by E. coli and Klebsiella pneumonia was 44. 1% and 43. 3%. The resistance of E. coli,Klebsiella pneumonia and Acinetobacter baumannii to carbapenems were 0. 7%,6. 6% and 61. 9%,respectively. Amikacin showed high antibacterial activity against negative gram bacilli isolates. No strains were found resistant to vancomycin or linezolid in positive gram cocci.Conclusion Gram-negative bacilli is the predominant isolate causing BSI in Jiamusi. E. coli and Klebsiella pneumonia are the most common pathogens causing bloodstream infections,and the Malta Brucella infection in this area should draw out attention.Amikacin is high sensitive to the isolates.
