复方甘草酸苷联合西药治疗儿童过敏性紫癜临床观察

摘　　要：目的:观察复方甘草酸苷联合西药治疗儿童过敏性紫癜的临床疗效。方法:将92例过敏性紫癜患儿分为观察组和对照组各46例。对照组采用西咪替丁注射液等西药对症治疗,观察组在对照组用药基础上给予静脉滴注复方甘草酸苷。观察2组相关炎症因子、免疫球蛋白及抗体水平的变化,评估临床疗效。结果:观察组总有效率为91.30%,高于对照组的71.74%,差异有统计学意义(P<0.05)。治疗后,2组干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)及白细胞介素-17(IL-17)水平均较治疗前降低(P<0.05),观察组的IFN-γ、IL-4及IL-17水平均比对照组下降更明显(P<0.05)。治疗后,2组IgA、C3水平均较治疗前降低(P<0.05),IgA/C3值均较治疗前减小(P<0.05);观察组的IgA、C3水平低于对照组(P<0.05),IgA/C3值小于对照组(P<0.05)。结论:复方甘草酸苷联合西药治疗儿童过敏性紫癜,可有效减轻炎症反应,改善患儿的免疫功能,效果优于单纯使用西药治疗。Objective： To observe the clinical effect of compound glycyrrhizin combined with western medicine for anaphylactoid purpura in children. Methods：Divided 92 cases of children with anaphylactod purpura into the observation group and the control group randomly, 46 cases in each group. Both groups were treated with cimetidine injection and other western medicine,while the observation group was additionally treated with intravenous injection of compound glycyrrhizin.Observed changes of level of relevant inflammatory factors,immunoglobulin（Ig） and antibody,and assessed clinical effect in both groups. Results：The total effective rate was 91.30% in the observation group,being higher than 71.74% in the control group（P0.05）. After treatment,levels of γ-interferon（IFN-γ）,interleukin-4（IL-4） and interleukin-17（IL-17） in both groups were decreased when compared with those before treatment（P 0.05）, and levels of IFN-γ, IL-4 and IL-17 in the observation group were reduced more obviously than those in the control group（P 0.05）. Comparing with those before treatment,levels of Ig Aand C3 in both groups after treatment were decreased（P0.05）,and value of IgA/C3 was declined（P0.05）. Levels of IgA and C3 in the observation group were lower than those in the control group（P0.05）,and value of IgA/C3 in the observation group was less than that in the control group（P 0.05）. Conclusion： The therapy of compound glycyrrhizin combined with western medicine for children with anaphylactoid purpura can effectively reduce inflammatory reaction and improve immune function of children. Its effect is superior to that of the therapy of pure western medicine.
