腹腔镜辅助阴式全子宫切除术在巨大子宫肌瘤治疗中的应用价值分析

摘　　要：目的分析腹腔镜辅助阴式全子宫切除术在巨大子宫肌瘤治疗中的应用价值,探究腹腔镜辅助的临床意义,以供参考。方法随机选取该院2014年11月—2016年11月该院收治的84例巨大子宫肌瘤患者作为研究对象,根据治疗方式的不同分为实验组和对照组,实验组42例患者采取腹腔镜辅助阴式全子宫切除术进行治疗,对照组42例患者采取传统开腹全子宫切除术,治疗结束后分析84例巨大子宫肌瘤患者的临床资料和治疗效果(手术时间、术中出血量及术后排气时间、下床时间、住院时间)。结果实验组术中出血量(64.35±5.41)mL少于对照组(98.65±6.74)mL,实验组患者肛门排气时间(0.87±0.19)d和住院时间(4.38±0.49)d均短于对照组的(1.64±0.21)d和(8.06±0.57)d,实验组平患者并发生发生5例,并发症发生率为11.90%,明显低于对照组的19.05%。两组比较差异有统计学意义(P<0.05)。结论对于巨大子宫肌瘤的治疗,采用腹腔镜辅助阴式全子宫切除术具有创伤小、疼痛小的优点,手术效果良好且并发症发生率低,值得在临床上推广应用。Objective To analyze the application value of laparoscopically assisted vaginal hysterectomy in treatment of giant hysteromyoma and study the clinical significance for reference. Methods 84 cases of patients with giant hysteromyoma admitted and treated in our hospital from November 214 to November 2016 were randomly selected and divided into two groups according to the treatment methods with 42 cases in each, the experimental group and control group respectively adopted the laparoscopically assisted vaginal hysterectomy and traditional total abdominal hysterectomy, and the clinical data and treatment effect（operation time, intraoperative bleeding amount, postoperative exhaust time, time to get out of bed and length of stay） of 84 cases were analyzed at the end of treat-ment. Results The intraoperative bleeding amount in the experimental group was less than that in the control group [（64.35±5.41）mL vs（98.65±6.74）mL], and the anus exhaust time and length of stay in the experimental group were shorter than those in the control group, [（0.87 ±0.19）d,（4.38 ±0.49）d vs（1.64 ±0.21）d,（8.06 ±0.57）d], and the incidence rate of complications in the experimental group was obviously lower than that in the control group（5 cases 11.90% vs 19.05%）, and the difference was obvious with statistical significance（P〈0.05）. Conclusion The laparoscopically assisted vaginal hysterectomy in treatment of giant hysteromyoma has the ad-vantages of small wound, less pains, good operation effect and low incidence rate of complications, and it is worth promotion and ap-plication in clinic.
