胃大部切除术治疗胃十二指肠溃疡大出血的临床疗效分析

摘　　要：目的分析胃大部切除术在胃十二指肠溃疡大出血中的治疗效果,并对其临床应用价值进行探究。方法回顾性分析所在医院2014年6月—2016年2月100例胃十二指肠溃疡合并大出血患者的临床资料,将其作为实验组,采用胃大部切除术进行治疗。按照1:1比例选取同期100例患者作为对照组,采用常规疗法,对两组患者临床治疗情况(治疗总有效率、总出血量、胃肠功能恢复时间、住院时间、并发症发生率)进行比较。结果实验组总有效率(96.0%)显著高于对照组(71.0%),两组差异有统计学意义(P<0.05);实验组总出血量、胃肠功能恢复时间、住院时间均显著低于对照组,两组差异有统计学意义(P<0.05);实验组并发症发生率为3.0%,明显低于对照组17.0%,组间差异有统计学意义(P<0.05)。结论在为胃十二指肠溃疡合并大出血患者提供临床诊疗服务时,采用胃大部切除术,可明显提高患者临床疗效,并在此基础上严格控制并发症,对患者预后具有促进效果。Objective To analyze the clinical curative effect of Subtotal gastrectomy in treatment of gastroduodenal ulcer hemorrhea.Methods 200 cases of patients with gastroduodenal ulcer hemorrhea admitted and treated in our hospital from June 2014 to February2016 were selected and divided into two groups with 100 cases in each, the experimental group and the control group were respective-ly treated with subtotal gastrectomy and routine therapy, and the total treatment effective rate, total bleeding amount, gastrointestinal function recovery time, length of stay and incidence rate of complications were compared between the two groups. Results There was an obvious difference in the total effective rate between the experimental group and the control group（96.0% vs 71.0%）（P0.05）, and the incidence rate of complications in the experimental group was obviously lower than that in the control group（3.0% vs 17.0%）, and the difference was statistically significant（P0.05）, and the total bleeding amount, gastrointestinal function recovery time and length of stay were obviously lower than those in the control group, and the differences were obvious（ P0.05）. Conclusion The clinical curative effect of Subtotal gastrectomy in treatment of gastroduodenal ulcer hemorrhea can be obviously improved and the complications can be strictly controlled on this basis, which has a promotion effect on the prognosis of patients.
