输卵管积水不同处理方式对冷冻胚胎移植临床结局的影响

摘　　要：目的探讨拟行冷冻胚胎移植的输卵管积水患者两种不同积水处理方式对助孕结局的影响。方法回顾分析2014年6月至2016月6月在四川省医学科学院·四川省人民医院拟行冷冻胚胎移植合并输卵管积水的132例患者的临床资料。根据不同积水处理方式分为腹腔镜组(n=64)及穿刺组(n=68),比较两组患者一般情况和助孕结局。结果两组患者一般情况差异无统计学意义(P>0.05),临床妊娠率(59.37%vs 44.12%)与胚胎着床率(30.15%vs 21.77)腹腔镜组高于穿刺组,但差异无统计学意义(P>0.05),早期流产率穿刺组高于腹腔镜组(20.00%vs 2.63%),差异有统计学意义(P<0.05)。结论若患者有优质胚胎,暂不考虑再次取卵的情况下,输卵管结扎/切除手术不失为拟行冷冻胚胎移植的输卵管积水患者较好的选择。Objective To study the effect of the two ways of hydrosalpinx treatment on the clinical outcome of frozen embryo transfer （FET）. Methods The clinical data of the 132 patients undergoing the frozen embryo transfer complicated with hydrosalpinx from June of 2014 to June of 2016 in Sichuan Provincial People＇s Hospital were analyzed retrospectively. The patients were divided into the laparoscopic group （n = 64） and the puncture group （n=68） according to the way of hydrosalpinx treatment. The baseline characteristics and clinical outcomes were compared between the two groups. Results There were no significant differences in the baseline characteristics between the two groups （P〉0.05）. The clinical pregnancy rate of the laparoscopic group （59.37%） was higher than that of the puncture group （44.12%） and the embryo implantation rate of the laparoscopic group （30. 15%） was higher than that of the puncture group （21.77%）, but the differences were not statistically significant （P〉 0.05）. The early abortion rate of the puncture group （20.00%） was significantly higher than that of the laparoscopic group （P〈0.05）. Conclusion If the hydrosalpinx patients with high-quality embryos don＇t have the plan of the egg retrieval for the time being, the tubal ligation or resection is a better choice.
