小型支撑钢板在Hoffa骨折中的效果观察及对患者膝关节功能的影响

摘　　要：目的探讨小型支撑钢板在Hoffa骨折中的效果及对患者膝关节功能、生活质量及疼痛程度的影响。方法收集2015年6月~2017年1月于我院骨科住院的采用小型支撑钢板治疗的80例Hoffa骨折患者作为本次的研究对象。采用HSS评分系统和VAS量表分别对患者手术前后膝关节功能及疼痛程度进行评估;统计患者术后膝关节功能优良率;采用SF-36量表评估患者手术前后生活质量变化。结果术后所有入组患者HSS量表总评分均较术前显著升高,VAS量表评分均较术前显著降低(P<0.01)。术后所有入组患者PF、RP、BP、GH、VT、SF、RE以及MH的SF-36量表评分项目均较术前明显升高(P<0.01)。80例入组患者均手术成功,且经Letenneur评估关节功能结果显示,优良73例,尚可5例,差2例,优良率为91.25%(73/80)。经随访均无术后并发症发生。结论对Hoffa骨折患者给予小型支撑钢板治疗可明显提高其术后膝关节功能,减轻术后疼痛,有助于患者生活质量的提高,安全可靠。Objective To investigate and observe the effect of small supporting plate in Hoffa fractures and its effect on patients＇ knee joint function, quality of life and degree of pain. Methods A total of 80 patients with Hoffa fractures who were admitted to the Department of Orthopedics in our hospital and were given small supporting plate from June 2015 to January 2017 were selected as the study subjects. The HSS scoring system and VAS scale were used to evaluate the knee joint function and pain degree before and after the surgery in the patients; the good and excellent rate of postoperative knee joint function was statistically analyzed; SF-36 scale was used to evaluate the changes of quality of life before and after surgery. Results The total score of HSS scale after the surgery was significantly higher than that before the surgery in all patients, and the score of VAS scale was significantly lower than that before the surgery（ P〈0.01）.Scores of SF-36 items of PF, RP, BP, GH, VT, SF, RE and MH in all the enrolled patients were significantly higher than those before the surgery（P〈0.01）. 80 enrolled patients were successfully given the surgery, and the results of joint function by Letenneur evaluation showed that 73 cases were good, 5 cases were moderate, and 2 cases were poor. The excellent and good rate was 91.25%（73/80）. There were no postoperative complications after follow-up visits. Conclusion Small supporting plate in the treatment of Hoffa fractures can significantly improve the function of knee joint after surgery, reduce postoperative pain, contribute to the improvement of quality of life of patients, and is safe and reliable.
