摘  要:目的探讨多部位穿刺标本的病理诊断结果及分析其影响因素。方法通过对我院2012年2月-2016年2月所做穿刺活检1261例标本做回顾性分析,并对可能影响穿刺活检内脏器官病理诊断的准确性的多种可能因素进行多元逐步Logistic回归分析。结果 1261例多部位穿刺标本的病理诊断结果情况:直接穿刺(目视下)50例,超声引导下穿刺661例,X线引导下穿刺320例,CT引导下穿刺150例,超声定位后的胸膜穿刺80例。以年龄、性别比例、穿刺部位、标本性质、结果病变性质、介导设备等因素为自变量进行多因素Logistic回归分析,结果示:病灶大、标本组织结构越完整诊断明确度越高。结论对器官组织穿刺活检的引导方法中,通过影像引导在临床中应用最广泛,且对于病灶大、标本组织结构完整的标本,其病理学诊断明确度越高。Objective To investigate the pathological diagnosis of multi-site puncture specimens and analyze the influencing factors.Methods A total of 1261 cases of biopsy collected in our hospital from February 2012 to February 2016 were retrospectively analyzed.Multivariate stepwise logistic regression analysis was performed on a variety of possible factors that could affect the accuracy of pathologic diagnosis of internal organs.Results Pathological diagnosis results of multi-site puncture specimens included 50 cases of direct puncture( visual),661 cases of ultrasound-guided puncture,320 cases of X-ray guided puncture,150 cases of CT-guided puncture,and 80 cases of ultrasound-targeted pleural puncture. Multivariate logistic regression analysis was performed of such independent variables as age,sex ratio,puncture sites,specimen nature,the results of pathological changes,mediated equipment and other factors. The results showed that a large lesion and complete tissue structure of the specimen could contribute to high accuracy of diagnosis.Conclusion The image-guided organ biopsy is the most widely used in clinical study and the pathological diagnosis rate is higher for specimens of a large size and with a complete tissue structure.
