负压封闭引流与臀大肌肌皮瓣转移联合用于骶尾部难治性褥疮修复中的效果

摘　　要：目的对骶尾部难治性褥疮患者采用负压封闭引流和臀大肌肌皮瓣转移术联合的方式进行治疗,查看其治疗效果。方法选择2015年1月至2017年1月期间在我院接受治疗的12例骶尾部难治性褥疮患者参与研究,对所有患者均采用负压封闭引流和臀大肌肌皮瓣转移术联合的方式进行治疗。对患者采用一期清创手术后,实行负压封闭引流1周至2周时间,并对患者进行相应的药物治疗和营养支持,二期则采用臀大肌肌皮瓣转移术进行修复,观察患者治疗的效果。结果经过一期手术治疗后,患者的创面均有了较好的恢复效果,患者的缺损组织处均存在新鲜肉芽组织,并且创面处未出现大量的脓性分泌物或者坏死组织,也未有患者出现严重感染。经过二期治疗后,所有患者的转移皮瓣均成功成活,患者的创面得到了满意的修复,对患者的生活未造成不利影响,生活质量评分和治疗前相比存在显著差异(P<0.05)。结论对骶尾部难治性褥疮患者采用负压封闭引流和臀大肌肌皮瓣转移术联合的方式进行治疗,可以有效改善患者的创面情况,促进患者早日康复,值得临床推广。Objective To treat the patients with sacrococcygeal refractory pressure ulcer with negative pressure occlusion and gluteus maximus myocutaneous fap for the treatment. Methods Twelve patients with sacrococcygeal decubitus decubitus who underwent treatment in our hospital from January 2015 to January 2017 were enrolled in this study. All patients were treated with negative pressure occlusion and gluteus maximus myocutaneous fap The way for treatment. After a period of debridement, the patients were treated with negative pressure for 1 week to 2 weeks, and the patients were treated with the corresponding drug treatment and nutritional support. The second stage was treated with gluteus maximus myocutaneous fap for repair. The efect of treatment. Results After one operation, the wound of the patients had better recovery efect. There was fresh granulation tissue in the defective tissue of the patients, and there was no large amount of purulent discharge or necrotic tissue at the wound surface. Patients with severe infection. After the second treatment, all patients with metastatic fap were successfully survived, the patient＇s wound was satisfactorily repaired, the patient＇s life did not adversely afect the quality of life score and before treatment there was a signifcant diference （P〈 0.05） The Conclusion The use of negative pressure occlusion and gluteus maximus myocutaneous fap transfer in the treatment of sacrococcygeal decompression bed sores can efectively improve the wound condition and promote the early rehabilitation of patients. It is worthy to be popularized.
