摘  要:目的观察龈交穴挑刺治疗Ⅰ、Ⅱ期内痔出血的临床疗效,并初步探讨其作用机制。方法将80例门诊Ⅰ、Ⅱ期内痔出血患者随机分为两组:挑刺组40例,挑刺龈交穴;对照组40例,予以太宁栓纳肛。治疗3d后、治疗7d后,分别观察两组临床疗效,包括便血及痔核黏膜情况;比较两组血小板(platelet,PLT)计数、凝血酶原时间(prothrombin time,PT)、凝血酶时间(thrombin time,TT)、活化部分凝血活酶时间(activated partial thromboplastin time,APTT)以及纤维蛋白原(fibrinogen,FBG)、血浆α-颗粒膜蛋白-140(granular membrane protein,GMP-140)、血栓素B2(thromboxane B2,TXB2)、6-酮-前列腺素F1α(6-keto-prostaglandin F1alpha,6-keto-PGF1α)的含量。结果挑刺组在改善患者出血症状方面明显优于对照组(P<0.05);两组治疗后PLT、PT、TT、APTT和FBG水平与治疗前比较,差异均无统计学意义(P>0.05)。与治疗前相比,治疗7d后挑刺组GMP-140、6-keto-PGF1α含量明显降低、TXB2含量显著升高,差异均有统计学意义(P<0.05);而治疗7d后对照组GMP-140、TXB2、6-keto-PGF1α含量无明显变化(P>0.05)。结论挑刺龈交穴能有效治疗Ⅰ、Ⅱ期内痔出血症状,其作用机制与其调节机体内GMP-140、TXB2和6-keto-PGF1α含量有关。Objective To investigate the clinical effect of pricking therapy at Yinjiao point in the treatment of stageⅠ/Ⅱinternal hemorrhoid hemorrhage and its mechanism of action.Methods A total of 80 patients with stageⅠ/Ⅱinternal hemorrhoid hemorrhage were randomly divided into pricking group(40 patients treated with pricking therapy at Yinjiao point)and control group(40 patients treated with Titanoreine Suppository).The clinical outcome was observed at 3 and 7 days after surgery,including hematochezia and hemorrhoid mucosa.The two groups were compared in terms of platelet count(PLT),prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT),fibrinogen(FBG),and plasma levels ofα-granular membrane protein-140(GMP-140),thromboxane B2(TXB2),6-keto prostaglandin F1 alpha(6-keto-PGF1α).Results The pricking group had a significantly greater improvement in bleeding symptoms than the control group(P<0.05),and there were no significant changes in PLT,PT,TT,APTT,and FBG after treatment in either group(P>0.05).After 7 days of treatment,the pricking group had significant reductions in GMP-140 and 6-keto-PGF1αand a significant increase in TXB2(P<0.05),while the control group had no significant changes in GMP-140,TXB2,and 6-ketoPGF1α(P>0.05).Conclusion Pricking therapy at Yinjiao point can effectively improve stageⅠ/Ⅱinternal hemorrhoid hemorrhage,possibly by regulating the levels of GMP-140,TXB2,and 6-keto-PGF1α.
