摘  要:目的研究CD8^+、CD4^+T细胞免疫功能与尖锐湿疣患者人乳头状瘤病毒(HPV)感染的相关性,观察卡介菌多糖核酸治疗后患者免疫功能的恢复状况。方法选取2015年10月至2016年10月经皮肤性病科确诊为尖锐湿疣的124例患者为研究对象(尖锐湿疣组),研究HPV感染与CD8^+、CD4^+T细胞的表达情况。同时选取健康体检的80例健康人员作为对照(健康成人组),124例尖锐湿疣患者采用电灼术进行清除,同期采用卡介菌多糖进行注射治疗并检测CD8^+、CD4^+T细胞比值。比较CD4^+/CD8^+比值与CD8^+、CD4^+T细胞值的差异;尖锐湿疣患者HPV感染采用核酸快速杂交基因分型进行研究检测。结果尖锐湿疣组CD4^+/CD8^+比值为33.10%±9.20%;CD3^+/CD4^+T细胞值为59.70%±10.10%、1.19±0.28,数据明显低于健康成人组44.20%±10.30%、69.40%±8.30%;1.67±0.24,两组数据比较差异显著(P<0.05);尖锐湿疣124例患者经脱落细胞样本中检测显示:HPV-DNA占比为95.97%(119/124),HPV6占比为47.06%阳性率较高;检出HPV-DNA阳性的119例尖锐湿疣患者采用卡介菌多糖核酸治疗后显示,CD8^+T细胞明显降低,外周血CD4^+/CD8^+数值与CD3^+、CD4^+T细胞比较显著升高,差异具有统计学意义(P<0.05)。结论尖锐湿疣患者随着机体免疫细胞失调下降,导致部分患者病情日渐加重,严重者会出现HPV免疫逃逸的情况,导致尖锐湿疣患者复发,临床采用卡介菌多糖核酸免疫调节剂治疗尖锐湿疣,可有效增强患者的自身免疫功能。Objective To study the CD8^+and CD4^+T lymphocyte immune function in patients with condyloma acuminatum and human papilloma virus( HPV) infection,and analysis of the status of recovery of immune function in patients with the clinical treatment of BCG-PSN.Methods From October 2015 to October 2016 in our hospital 124 Condyloma Acuminatum patients were selected as the research object,HPV and the expression of CD8^+and CD4^+T lymphocytes in the human body were analyzed. At the same time,80 healthy cases in our hospital for health examination were selected as controls. 124 cases of patients with condyloma acuminatum performed electrocautery removal,simultaneously,BCG polysaccharide injection and CD8^+and CD4^+T lymphocyte ratio were detected. The CD4^+/CD8^+ratio and the difference of CD8^+and CD4^+T lymphocytes were compared. The HPV infection of condyloma acuminatum patients was detected by nucleic acid rapid hybridization genotyping.Results CD4^+/CD8^+of Condyloma Acuminatum Patients was 33. 10% ± 9. 20%; CD3^+/CD4^+T lymphocyte value was 59. 70% ± 10. 10%,1. 19 ± 0. 28,the data was significantly lower than that of healthy adult group 44. 20% ± 10. 30%,69. 40% ± 8. 30%,1. 67 ± 0. 24 with significant difference( P〈0. 05). 124 cases of Condyloma Acuminatum Patients by exfoliated cell samples display HPV-DNA accounted for 95. 97%( 119/124),HPV6 accounted for a higher positive rate 47. 06%; the positive rate of HPV-DNA in 119 cases of patients with condyloma acuminatum by BCG-PSN after treatment showed that CD8^+T lymphocytes decreased,peripheral blood CD4^+/CD8^+and CD3^+,numerical CD4^+T lymphocytes were significantly higher,the difference was statistically significant( P〈0. 05). Conclusion Immune cell function in patients with condyloma acuminatum decreased,resulting in aggravating of some patients and recurrence.
