中性粒细胞/淋巴细胞比值与2型糖尿病病人下肢动脉粥样硬化的相关性分析

摘　　要：目的:探讨中性粒细胞/淋巴细胞比值(NLR)与2型糖尿病(T2DM)病人下肢动脉粥样硬化的相关性。方法:选取T2DM病人53例,均行彩色多普勒超声下肢血管检查,根据有无病变将病人分为下肢动脉粥样硬化病变组(病变组)36例与非下肢动脉粥样硬化病变组(非病变组)17例,其中病变组根据病变程度又分为A组(毛糙)、B组(增厚或斑块)、C组(增厚+斑块)3个亚组,比较各组相关临床指标及NLR,并进行logistic多元回归分析。结果:病变组糖化血红蛋白、总胆固醇(TG)、C反应蛋白(CRP)水平和中性粒细胞计数、淋巴细胞计数及NLR均显著高于非病变组(P<0.01)。病变组中,3个亚组间NLR差异有统计学意义(P<0.01),其中C组NLR显著高于A组和B组(P<0.01),B组亦高于A组(P<0.05)。Logistic回归分析显示,TG、CRP和NLR均为T2DM病人下肢动脉粥样硬化的独立危险因素(P<0.05)。结论:TG、CRP和NLR均与T2DM病人下肢动脉粥样硬化密切相关,NLR对该类病变有预测价值。Objective：To investigate the association between neutrophil/lymphocyte ratio（NLR） and lower limb atherosclerosis in patients with type 2 diabetes mellitus（T2DM）. Methods：Fifty-three patients with T2DM whose lower limb blood vessel was examined using color Doppler ultrasono graphy were divided into the lesion group（36 cases with lower limb atherosclerosis） and nonlesion group （17 cases without lower limb atherosclerosis）. The lesion group was subdivided into the group A（crude）,group B（thickening or plaques） and group C（ thickening and plaques） according to the pathological change. The correlated clinical indexes and NLR between these groups were compared and analyzed using logistic regression analysis. Results：The levels of glycosylated hemoglobin,triglyceride （ TG） and C-reative protein（ CRP） ,neutrophil count,lymphocyte count and NLR in lesion group were significantly higher than those in nonlesion group（P〈0. 01）. The differences of NLR between A,B and C groups were statistically significant（P〈0. 01）,the NLR in group C was significantly higher than that in group A and group B（P〈0. 01）,and the NLR in group B was higher than that in group A （P〈0. 05）. Logistic multivariate regression analysis showed that the TG,CRP and NLR were the independent risk factors of lower limb atherosclerosis in patients with T2DM（P 〈0. 05）. Conclusions：The TG,CRP and NLR are closely correlated with the lower limb atherosclerosis in patients with T2DM,and the NLR has predictive values in diagnosing the disease.
