持续正压通气治疗阻塞性睡眠呼吸暂停低通气综合征167例临床回顾性分析

摘　　要：目的调查持续正压通气(CPAP)治疗阻塞性睡眠呼吸暂停低通气综合征(OSAS)患者的依从性及其影响因素,并比较CPAP与其他疗法的有效性。方法对2012年9月至2014年6月医院就诊,并经多导睡眠监测和临床症状诊断为中度以上[呼吸暂停低通气指数(AHI)>15]的OSAS患者进行电话随访获取资料,并分析CPAP治疗的依从性与影响因素,比较CPAP与行为学疗法对OSAS治疗的有效性。结果在所有需要CPAP治疗的167例患者中,仅23例(13.77%)坚持使用了CPAP治疗,大部分患者对OSAS及CPAP的相关知识匮乏;影响患者治疗依从性的因素主要有年龄、体质量指数、文化程度、OSAS对生活的影响程度、患者对OSAS的了解程度(P<0.05);行为学疗法与CPAP治疗对OSAS均有效,但后者的好转率明显高于前者(P<0.01)。结论 CPAP治疗的依从性较低,年龄、体质量指数、文化程度、OSAS对生活的影响程度、患者对OSAS的了解程度是影响依从性的主要因素。CPAP治疗的好转率明显高于行为学治疗。Objective To investigate the compliance and influence factors of continuous positive airway pressure （ CPAP ） for treating obstructive sleep apnea hypopnea syndrome（OSAS）, and compare the efficacy of the CPAP and other treatments. Methods Clinical data was got by telephone follow-up on moderate OSAS patients （ AHI 〉 15 ） diagnosed by polysomnography and clinical symptoms, who were admitted to our hospital from September 2012 to June 2014, the compliance and influence factors of CPAP were analyzed, the effective-ness of CPAP and behavioral therapy in the treatment of OSAS was compared. Results In 167 patients who required CPAP treatment, only 23 patients （ 13. 77%） persisted in the use of CPAP. Most patients lack of knowledge about OSAS and CPAP. The main factors influencing compliance were age , BMI , educational level , the influence of OSAS on life and the understanding degree of OSAS （ P 〈 0. 05 ） . Behavioral therapy and CPAP treatment were effective for OSAS, but the improvement rate of the latter was significantly higher than that of the former （ P 〈 0. 01 ） . Conclusion Patients treated with CPAP had lower compliance, the main factors influencing compliance were age, BMI, educational level, the influence of OSAS on life and the understanding degree of OSAS. The improvement rate of CPAP treatment is obviously higher than that of behavioral therapy.
