DRGs在重庆某三甲医院住院绩效管理中的应用

摘　　要：目的通过应用DRGs相关指标,对重庆市某三甲医院近5年住院医疗服务绩效进行评价,为医院住院医疗服务的精细化管理提供借鉴。方法以北京版DRGs(BJ-DRGs)分组方式,运用CMI、DRGs组数、费用消耗指数、时间消耗指数、低风险死亡率等指标对2012—2016年住院医疗服务绩效进行评价。结果 2012—2016年研究医院DRGs组数逐渐增多;CM I值呈下降趋势,但2016年有所回升;低风险死亡率下降;费用消耗指数呈增加趋势,但2016年有所回落;时间消耗指数逐年下降。研究医院前3位MDC疾病专业为呼吸、新生儿、消化。结论基于DRGs相关指标分析近5年住院医疗服务绩效,能够客观评价医院及其临床科室的服务广度、技术难度、服务效率以及诊疗安全质量情况。Objective: Through the application of DRGs related indicators,the performance evaluation of medical service in a hospital of Class A,Grade Three in Chongqing is evaluated,w hich can be used as a reference for the detailed management of hospitalization service. Method: Based on the Beijing version of DRGs( BJ-DRGs),the hospitalization performance of medical service in2012-2016 w as evaluated by using CM I DRGs group,cost consumption index,time consumption index,low risk mortality and other indicators. Result: The number of DRGs group gradually increased in the research hospital from 2012-2016. CM I value show ed a dow nw ard trend,but it recovered in 2016. Low er risk mortality decreased. The cost consumption index is increasing,but it has fallen back in 2016. Time consumption index decreased year by year. And the first three M DC diseases of respiration,new born and digestion w ere studied in the hospital. Conclusion: Based on the related indexes of DRGs,the performance of hospitalization service in the last five years can be used to objectively evaluate service breadth,technical difficulty,service efficiency,and safety and quality of diagnosis and treatment.
