摘 要:目的了解济南市1~14岁儿童血清乙肝表面抗体(Anti-HBs)水平,为进行儿童Anti-HBs水平监测及加强免疫决策提供依据。方法选取2014年5月—2017年3月在济南市儿童医院健康查体的1~14岁儿童878例作为研究对象。抽取所有儿童的静脉血,采用化学发光法检测血清Anti-HBs水平。结果 878例1~14岁儿童血清Anti-HBs阳性有639例,阳性率为72.78%。其中,10~100 mIU/ml有252例(28.70%),大于100 mIU/ml有387例(44.08%)。女童血清Anti-HBs阳性率为78.24%(284/363),男童的阳性率为68.93%(355/515),差异具有统计学意义(χ~2=9.31,P=0.002)。不同年龄组间儿童血清HBs-Ab阳性率不同,差异具有统计学意义(χ~2=56.27,P<0.001)。其中1~2岁儿童Anti-HBs阳性率最高,而后逐渐降低,至6~7岁儿童Anti-HBs阳性率最低,而7~10岁儿童Anti-HBs阳性率又呈上升趋势。结论儿童接种乙肝疫苗后随着年龄的增长HBs-Ab阳性率逐渐下降,建议加强儿童乙肝表面抗体的免疫监测,以便及时加强免疫。Objective To investigate the level of Anti-HBs of children aged from 1 to 14 years old in Ji'nan City, and provide evidence for monitoring the Anti-HBs levels and formulating the strategies for booster immunization. Methods A total of 878 children aged 1-14 years from May 2014 to March 2017 in Ji'nan children's hospital were selected as the study subjects. Venous blood samples were taken from all children and serum Anti-HBs levels were detected by chemiluminescence method. Results 878 cases of 1-14 years old children serum Anti-HBs positive in 639 cases, the positive rate was 72.78%. Among them, 10-100 mIU/ml had 252 cases (28.70%), there were 387 cases (44.08%) greater than 100 mIU/ml. The positive rate of serum Anti-HBs in girls was 78.24% (284/363), the positive rate of boys was 68.93% (355/515), the difference was statistically significant (x^2=9.31, P=-0.002). The positive rates of serum HBs-Ab in children of different age groups were different, the difference was statistically significant (x^22=56.27, P 〈 0.001). The positive rate of Anti-HBs in children aged 1-2 years was the highest, and then decreased gradually. The lowest positive rate of Anti- HBs in children aged 6-7 years old. However, the positive rate of Anti-HBs in children aged 7-10 years was on the rise. Conclusion The positive rate of serum anti-HBs was decreased in children with the increase of age after basic vaccination. It is suggested that monitoring of the anti-HBs levels should be strengthened for the implementation of supplementary HBV vaccination.
