腹横肌平面阻滞联合静脉镇痛的临床应用

摘　　要：目的分析对剖宫产患者术后给予腹横肌平面阻滞(TAP)、静脉联合镇痛的作用。方法选取本院妇产科2016年6月—2017年6月收治的60例足月产妇作为研究对象。所有产妇拟行剖宫产术。根据术后不同镇痛模式,将产妇分为联合组、静脉组,每组各30例。联合组给予TAP、静脉联合镇痛,静脉组单独采用静脉自控镇痛(PCIA)。记录两组产妇术后2 h、12 h、24 h视觉模拟疼痛评分(VAS)、术后24 h镇痛药物使用量及镇痛泵按压次数、不良反应、产妇满意度评分情况。结果术后24 h联合组产妇的VAS评分、镇痛药物使用量、镇痛泵按压次数、不良反应发生率均低于静脉组,且满意度明显更高,差异均具有统计学意义(P<0.05)。结论 TAP、PCIA联合镇痛对剖宫产产妇术后镇痛作用明显,减少了镇痛药物使用量,不良反应少。Objective To analyze the transversus abdominis plane block in patients after cesarean section （TAP） combined with intravenous analgesia. Methods 60 cases of full-term pregnant women who were treated in our department from June 2016 to June 2017 were selected as the subjects. All women were scheduled to undergo caesarean section. According to different postoperative analgesia patterns, the parturients were divided into combined group and intravenous group, with 30 cases in each group. The combined group was given TAP and intravenous analgesia, and the intravenous group was given intravenous controlled analgesia （PCIA） alone. 2 h, 12 h, 24 h visual analogue pain scores （VAS）, postoperative 24 h analgesic drug use, analgesic pump press times, adverse reactions, and maternal satisfaction scores were recorded in two groups. Results At 24 h after operation, the VAS score, the amount of analgesic drug use, the times of analgesic pump pressing and the incidence of adverse reactions in the combined group were lower than those in the intravenous group, and the degree of the satisfaction was significantly higher, the differences were statistically significant （P 〈 0.05）. Conclusion Combined analgesia with TAP and PCIA has obvious analgesic effect on cesarean section, reduces the use of analgesic drugs, and has less adverse reactions.
