摘 要:目的探讨Carto和NavX三维标测系统指导房室结折返性心动过速消融治疗的安全性和有效性差异。方法选取2016年9月—2017年5月于苏州大学附属第一医院心内科经电生理检查证实为房室结双径路的患者73例作为研究对象。按照标测系统,将其分为Carto组(35例)与NavX组(38例),比较两组患者的手术成功率、并发症发生率、手术时间、X线曝光时间及随访复发率。结果所有患者均术后即刻成功,无手术相关严重并发症。Carto组患者的X线曝光时间短于NavX组,差异具有统计学意义(P<0.05)。Carto组患者的手术时间长于NavX组,差异具有统计学意义(P<0.05)。术后随访31~306天,平均随访时间为(152.5±66.0)天,Carto组与NavX组均无复发病例,两组患者的手术复发率对比,差异无统计学意义(P>0.05)。结论两种主流三维标测系统指导AVNRT消融治疗均安全、有效,Carto系统X线曝光时间较NavX系统短,但手术时间延长。Objective To investigate the differences in efficacy and safety between two prevalent three-dimensional mapping systems in the catheter ablation of atrioventricular nodal reentrant tachycardia (AVNRT). Methods 73 patients with dual atrioventricular nodal pathways proved by electrophysiological examination from September 2016 to May 2017 in the Department of Cardiology of First Hospital Affiliated to Soochow University were selected as subjects. According to the mapping system, they were divided into Carto group (35 cases) and group NavX (38 cases). The operative success rate, complication rate, operation time, X-ray exposure time and recurrence rate were compared between the two groups. Results All patients were successful immediately after operation without severe complications related to operation. The X-ray exposure time of group Carto patients was shorter than that of NavX group, the difference was statistically significant (P〈0.05). The operation time of the Carto group was longer than that of NavX group, the difference was statistically significant (P〈0.05). The patients were followed up for 31 - 306 days. The average follow-up time was (152.5±66.0) days. There was no recurrence in the Carto group and the NavX group, and there was no significant difference in the operative recurrence rate between the two groups (P〉0.05). Conclusion Both three- dimensional mapping systems are effective and safe in AVNRT ablation therapy. The application of Carto system was associated with shorter fluoroscopy time but more procedure time.
