高频彩超在婴幼儿复发性肠套叠诊疗中的应用研究

摘　　要：目的探讨高频彩超对婴幼儿复发性肠套叠的诊疗价值。方法回顾性分析我院收治的140例复发性肠套叠患儿的临床资料,根治治疗方法不同分为两组,各70例。对照组行X线透视下灌肠复位,观察组行高频彩超下灌肠复位,观察患儿高频彩超声像图特征及诊断准确率,并对比两组的复位效果、并发症发生情况等。结果高频彩超检出率为98.57%(69/70);与对照组比较,观察组复位成功率明显升高(P<0.05),惊厥、高热、肠穿孔等并发症发生率明显降低(P<0.01),复位成功后住院时间明显缩短(P<0.01)。结论高频彩超可对婴幼儿复发性肠套叠准确诊断,且其指导下灌肠复位整复成功率高、并发症少,值得应用推广。Objective To explore the treatment values of high frequency color Doppler ultrasonography in the diagnosis and treatment of recurrent intussusception in infants. Methods One hundred and forty cases of children with recurrent intussusception who were diagnosed and treated in outpatient department of pediatrics in were selected,and were grouped by the random number table method. The control group were under X-ray fluoroscopy enema reduction,while the experimental group were treated with enema under high frequency color Doppler ultrasound. The ultrasonographic features and diagnostic accuracy of recurrent intussusception in infants with high frequency color Doppler ultrasonography were observed,and the effect of reduction was compared between the two groups. Results The detection rate of high frequency color Doppler ultrasound was 98. 57% （69/70）; Compared with the control group,the success rate of reduction in the experimental group was significantly increased,the incidence of complications such as intestinal perforation,convulsions,high fever were decreased significantly,and the hospitalization time was significantly shorter after the reduction of success （P 〈0. 01）. Conclusion The high frequency color Doppler ultrasound can be used for accurate diagnosis of recurrent intussusception in infants,and under the guidance of the enema,which has high success rate and fewer complications,it is worthy to be applied and promoted.
