健脾理气中药治疗胃食管反流病疗效及对食管括约肌静息压、酸反流水平的影响

摘　　要：目的探讨健脾理气中药治疗胃食管反流病疗效及对食管括约肌静息压、酸反流水平的影响。方法将120例胃食管反流病患者随机分为2组,对照组60例给予西药治疗,观察组60例在此基础上加用健脾理气中药治疗,观察2组治疗前后中医证候积分、RDQ量表评分、食管测压水平、24 h食管pH水平及SF-36评分变化情况,统计2组近期疗效和不良反应发生情况。结果 2组治疗后反酸嗳气、胃脘胀满、双胁疼痛、胸闷、食欲不振及大便不畅积分均显著低于治疗前(P均<0.05),且观察组治疗后各项积分均显著低于对照组(P均<0.05);2组治疗后反酸、反流、烧心及胸痛评分均显著低于治疗前(P均<0.05),且观察组治疗后各项评分均显著低于对照组(P均<0.05);2组治疗后食管测压水平、24 h食管pH水平及SF-36评分均显著优于治疗前(P均<0.05),且观察组治疗后各项指标水平均显著优于对照组(P均<0.05);观察组近期治疗总有效率显著高于对照组(P<0.05),随访复发率显著低于对照组(P<0.05)。结论健脾理气中药治疗胃食管反流病可有效缓解临床症状体征,改善食管测压和24 h食管pH水平,提高日常生活质量,且有助于降低复发风险。Objective It is to explore the effect of Jianpi Liqi herbs on gastric＇esophageal reflux disease and on esophageal sphincterstatic pressure and acid reflux. Methods 120 cases of patients with gastroesophageal reflux disease were randomly di- vided into 2 groups： the control group （n = 60） treated with western medicine and the observation group （n = 60） treated with Jianpi Liqi Chinese medicine on the basis of therapy in the control group. The changes of scores of TCM syndrome and RDQ rating scale, esophageal pressure, 24 h esophageal pH levels and scores of SF-36 scale before and after treatment were ob- served, the recent curative effects were evaluated and the occurrences of side effects were recorded in both group. Results Af- ter treatment, the scores of belching, fullness or distension in the stomach, double side pain, chest tightness, poor appetite and inhibited defecation were significantly lower than those before treatment in both groups（P 〈 0. 05） , and the scores in the observation group were significantly lower than those in the control group （P 〈 0.05 ） ; the scores of anti-acid, reflux, heart- burn and chest pain were significantly lower than those before treatment in both groups （ P 〈 0.05 ） , and all scores were signifi- cantly lower in the observation group than those in the control group （ P 〈 0.05 ）. After treatment, the esophageal test pressure level, 24 h esophageal pH level and SF-36 scores were significantly better than those before treatment in both groups （P 〈 0.05） , and the improvements of these indicators in the observation group were significantly better than those in the control group （P 〈 0.05）. The recent total effective rate in the observation group was significantly higher and the following-up rate was significantly lower than that in the control group （P 〈 0. 05）.
