摘  要:目的观察并探讨α-硫辛酸辅助基础胰岛素治疗2型糖尿病临床作用。方法选取我院于2015年1月至2016年3月收治的100例2型糖尿病患者作为研究对象,随机分为实验组与对照组,对照组患者则采取常规胰岛素降糖药物治疗,实验组患者在此基础上加用α-硫辛酸抗氧化治疗,比较两组治疗前后空腹血糖、血脂指标、氧化应激等指标变化,评估两组治疗效果及不良反应。结果治疗4周后,实验组空腹血糖显著低于对照组(6.18±0.88 vs.6.71±1.02mmol/L),差异具有统计学意义(P<0.05);两组治疗后血脂指标差异均无统计学意义(P>0.05);实验组患者治疗后尿蛋白排泄率、血浆内皮素-1、一氧化氮水平(125.84±15.51 mg/24h、57.13±10.21ng/L、33.53±5.11μmol/L)显著低于对照组(146.12±18.13 mg/24h、72.25±12.87 ng/L、40.79±7.32μmol/L);实验组治疗后丙二醛含量显著低于对照组(3.28±1.09μmol/L vs.4.48±1.13μmol/L),而超氧化物歧化酶含量显著高于对照组(44.54±7.48U/mL vs.32.57±6.94 U/mL),差异具有统计学意义(P<0.05);所有患者在接受治疗过程中未发生不良反应,实验组治疗后临床有效率(94.12%vs.77.55%)显著高于对照组(P<0.05)。结论氧化应激在2型糖尿病发生发展中具有重要作用,α-硫辛酸辅助基础胰岛素治疗2型糖尿病可以有效的改善患者的病情。Objective To investigate the efficacy of α-lipoic acid combined with basic insulin therapy in treatment of type 2 diabetes mellitus. Methods 100 patients with type 2 diabetes mellitus from Jan 2015 to Mar 2016 were selected as the subjects and randomly divided into experimental group and control group, which the control group were treated with basic insulin therapy, and in addition, the experimental group were added with α-lipoic acid therapy. The fasting fasting plasma glucose ,blood lipid parameters and oxidative stress markers before and after treatment were compared, and adverse drug reactions and efficacy were evaluated after treatment. Results 4 weeks after treatment, The fasting blood glucose in experimental group was significantly lower than that of the control group (6.18±0.88 mmol/L vs. 6.71±1.02 mmol/L) (P〈0.05); the differences on serum lipid indexes after the treatment were not statistically significant between the two groups (P 〉 0.05); urinary protein excretion rate in the experimental group(125.84±15.51 mg/24 h vs. 146.12±18.13 mg/24 h), endothelin -1 (57.13±10.21 ng/L vs. 72.25±12.87 ng/L) and nitric oxide (33.53±5.11μmol/L vs.40.79±7.32μmol/L) level after treatment were all significantly lower than that of the control group(P〈0.05); MDA in experimental group after treatment was significantly lower than that of the control group (3.28±1.09μmol/L vs. 4.48±1.13μmol/L) μmol/L, and superoxide dismutase were significantly higher than those in control group(3.28±1.09μmol/L vs. 4.48±1.139μmol/L) (P〈0.05); all the patients had no adverse reactions during the treatment process, the clinical effective ratio of experimental group after treatment was significantly higher than that of the control group(94.12% vs.77.55%) (P 〈 0.05). Conclusion oxidative stress plays an important role in the development and progression of type 2 diabetes mellitus, and α-lipoic acid therapy can be effective for improving the patient's condition
