血尿康联合低分子肝素钙治疗小儿邪毒阻络型过敏性紫癜性肾炎疗效观察

摘　　要：目的观察血尿康联合低分子肝素钙治疗小儿邪毒阻络型过敏性紫癜性肾炎的临床疗效。方法将62例邪毒阻络型过敏性紫癜性肾炎患儿随机分为观察组和对照组。2组均给予醋酸泼尼松片、雷公藤多苷片口服,低分子肝素钙皮下注射,观察组在此基础上给予自拟血尿康口服,2组均以2周为1个疗程。治疗2个疗程后,观察2组临床症状消失时间及治疗前后肾功能指标、体液免疫指标及血清C反应蛋白(CRP)、D-二聚体(D-D)及白细胞介素-8(IL-8)水平变化情况。结果治疗后,观察组皮肤紫癜、关节疼痛、腹痛及蛋白尿消失时间均显著短于对照组(P均<0.05);治疗后,2组尿微量白蛋白(m ALB)、β2-微球蛋白(β2-MG)、24 h蛋白尿(24h Upro)及免疫球蛋白(Ig A)、免疫球蛋白E(Ig E)、免疫球蛋白G(Ig G)、CRP、D-D、IL-8含量均显著降低(P均<0.05),且观察组治疗后各指标水平均显著低于对照组(P均<0.05)。结论血尿康联合低分子肝素钙治疗小儿邪毒阻络型过敏性紫癜性肾炎疗效确切,能够明显改善患儿体液免疫紊乱和机体炎症反应。Objective It is to observe the clinical effect of combination of Xueniaokang and low molecular weight heparin calium on infantile Henoch-Schonlein purpura nephritis with pattern of collateral stasis by toxin pathogen. Methods 62 cases of infantile Henoch-Schonlein purpura nephritis with pattern of collateral stasis by toxin pathogen were randomly divided into ob- servation group and control group. Both groups were given prednisone acetate tablets, tripterygium wilfordii glycosides tablets by oral administration, low molecular heparin calcium by subcutaneous injection, additionally the observation group was given Xueniaokang orally. Two weeks as one treatment course,after two courses, the disappearing time of clinical symptoms and the changes of indexes of renal function and humoral immunity and the levels of serum C-reactive protein （CRP） , D-dimer （D-D） and interleukin 8 （IL-8） before and after treatment were observed in both groups. Results After treatment, the disappearing time of skin purpura, joint pain, abdominal pain and proteinuria in the observation group was significantly shorter than that in the control group（P 〈 0.05 ） ; the levels of mALB, [~2-MG, 24 hUpro and IgA, IgE, IgG, CRP, D-D, IL-8 were significantly lower than that before treatment in both groups （ P 〈 0.05 ） , and the levels were more lower in the observation group after treat- ment than those in the control group （ P 〈 0.05 ）. Conclusion Xuenlaokang combined with low molecular weight heparin ealium can obviously improve the fluid immunity and inflammation in children with Henoch-Schonlein purpura nephritis with pattern of collateral stasis by toxin pathogen, the curative effect is definite.
