摘 要:目的初步探讨颈围与冠状动脉粥样硬化性心脏病(冠心病)的相关性。方法连续选取接受冠状动脉造影检查的患者680例,测量颈围等体脂指标,记录临床资料、辅助检查及冠状动脉造影结果。用t检验、Logistic回归分析等方法,分组分性别研究比较颈围与冠心病的相关性。结果在男性,颈围并非冠心病的独立预测因子,而吸烟、高血压病则有统计学意义(P均<0.05)。在女性,年龄、颈围、血糖异常是冠心病的独立预测因子(P均<0.05)。在男性,校正了年龄后,颈围从25%以下增加至25%~50%时,冠心病对颈围的OR值为2.57,而再增加颈围四分位区间时,OR值反而有所降低。进一步校正吸烟、疾病和腰臀比,仍然得出相似的趋势。而在女性,校正了年龄后,冠心病患病风险随颈围四分位逐步增加。进一步校正吸烟、疾病和腰臀比,仍然得出相似的趋势。结论颈围与冠心病密切相关。在男性,颈围与冠心病非简单线性关系,颈围在36.38~37.80cm的人群的冠心病发病风险最高;在女性,颈围在35.35 cm以上的人群的冠心病发病风险最高。Objective To preliminarily investigate the correlation between neck circumference and coronary atherosclerotic heart disease( coronary heart disease). Methods In total,680 consecutive patients receiving coronary angiography were recruited in this study. Neck circumference and other body fat parameters were measured. Clinical data,auxiliary examination and coronary angiography results were recorded. All participants were divided into two groups according to gender. The correlation between neck circumference and coronary heart disease was analyzed. Results For the male,neck circumference was not an independent risk factor of coronary heart disease,whereas smoking,hypertension and waist to hip ratio were significantly correlated with the incidence of coronary heart disease( P < 0. 05). For the female counterparts,age,neck circumference and abnormal glucose level were the independent risk factors of coronary heart disease( P < 0. 05). After the age adjustment for the male,the OR between coronary heart disease and neck circumference was calculated as 2. 57 when the neck circumference was increased from < 25% to 25%-50%. The OR was declined when the neck circumference was increased by inter-quartile range. A similar tendency was obtained after the adjustment of smoking,disease and waist to hip ratio. For the female counterparts,the risk of coronary heart disease was gradually elevated along with the inter-quartile range of neck circumference. A resembling tendency was achieved after the adjustment of smoking,disease and waist to hip ratio. Conclusions Neck circumference is significantly correlated with the risk of coronary heart disease. For the male,the linear relationship is not observed between neck circumference and coronary heart disease. The incidence of coronary heart disease is the highest among the population with neck circumference between 36. 38 and 37. 80 cm. For the female,the risk of coronary heart disease is the highest in those with neck circumference above 35. 35 cm.
