彩色多普勒超声对静脉注射海洛因人员下肢静脉血流动力学改变的诊断价值

摘　　要：目的应用彩色多普勒超声观察静脉注射海洛因人员下肢静脉血流动力学改变,探讨其临床应用价值。方法在安徽省南湖强制隔离戒毒所内具有2年或以上的海洛因静脉注射史的戒毒人员中随机选取59例为观察组,在同期中国解放军第105医院健康体检的自愿者中抽取52例无吸毒史的自然人群为对照组,比较两组股总静脉内径、最大流速(Vmax)、瘀滞指数(SI),并观察血栓发生状况及静脉瓣膜功能。结果观察组患者股总静脉内径(5.65±1.61)mm低于对照组的(6.97±0.93)mm,流速(30.06±11.75)cm/s低于对照组的(39.33±13.58)cm/s,瘀滞指数(1.34±0.97)高于对照组的(0.98±0.54)、血栓发生率15.25%高于对照组的3.85%、静脉瓣膜功能不全发生率13.56%高于对照组的1.92%,差异均有统计学意义(P<0.05)。结论彩色多普勒超声检查可较为敏感地评价血液瘀滞状态,对预测血栓形成风险有一定价值。Objective To evaluate the value of color doppler ultrasonography in the diagnosis of venous hemodynamics in the lowerextremity in intravenous injection of heroin. Methods Fifty - nine patients were randomly selected from drug addicheroin intravenous injFifty - nine patients were randomly selected from drug addicts who had a history of heroin intravyears or more in the Nanhu Compulsory Detoxification Center in Anhui Province as case group. In the same period,fifty - two volunteers of non - drug addicts were selected inthe physical examination in 105th Hospital of PLA as control group. The femoral vein diameter,maximal velocity （ Vmax） and stasis index （ SI） of the two groups were compared. The thrombosis status and venousults The mean femoral vein diameter [（5.65 ±1.61） mm] wasless,Vmax [ [30. 06 ± 11.75） cm /s] was slower, SI （ 1. .4 ± 0. 97） washigher,the rate of thrombosis （ 15. 25% ）was higher,andthe rate of venous insufficiency in the lower extremities （ 13. 56% ） was higher than those in control group [ （6. 97 ±0. 93 ） mm, （ 39. 33 ± 13. 58 ） cm/s, （ 0 . 9 8 ± 0 . 5 4 ）,3 .8 5% ,1 .9 2% ] . There were significant differ-ences between case group and control group （P 〈0. 05） . Conclusion Color Doppler Ultrasonography can be seblood stasis,which is valuable for predicting the risk of thrombosis.
