骨折致桡神经损伤两种内固定术对照研究

摘　　要：目的对比锁定加压钢板(LCP)与交锁髓内钉内固定术治疗肱骨干骨折并发桡神经损伤的临床效果。方法选择2013-05—2016-10开封市第二人民医院收治的肱骨干骨折并发桡神经损伤患者84例,根据随机数字表法分为2组各42例。对照组实施LCP内固定术,观察组实施交锁髓内钉内固定术。比较2组各项手术指标、肩关节功能、肘关节功能、桡神经损伤恢复程度。结果对照组术中出血量、手术用时、骨折愈合时间、住院时间、骨线消失时间均高于观察组,差异有统计学意义(P<0.05);对照组肩关节功能优良率、肘关节功能优良率均低于观察组,差异有统计学意义(P<0.05);与对照组相比,观察组桡神经损伤恢复程度更优,差异有统计学意义(P<0.05)。结论与LCP内固定术相比,对肱骨干骨折并发桡神经损伤患者实施交锁髓内钉内固定术治疗效果更优,能优化各项手术指标,改善肩关节功能、肘关节功能,提高桡神经损伤恢复程度,值得临床推广。Objective To investigate the clinical effects of locking compression plate(LCP)and internal fixation with interlocking intramedullary nail in treatment of humeral shaft fracture combined with radial nerve injury.Methods Eighty-four patients with humeral shaft fracture combined with radial nerve injury in the Second People's Hospital of Kaifeng from May 2013 to October 2016 were selected,and they were divided into two groups by radnom number table,42 cases in each group.Control group was given LCP internal fixation,while observation group was given interlocking intramedullary nail internal fixation.The surgical indexes,shoulder's function,elbow joint function,recovery degree of radial nerve injury were compared between the two groups.Results The intraoperative bleeding volume,operation time,fracture healing time,hospital stays,disappearance time of bone line in control group were higher than those in observation group(P<0.05).The excellent rate of shoulder joint and elbow joint function in control group were lower than those in observation group(P<0.05).Compared with control group,the recovery degree of radial nerve injury in observation group was better(P<0.05).Conclusion Compared with LCP internal fixation,interlocking intramedullary nail internal fixation has better curative effects,it can optimize surgical indexes,improve shoulder joint and elbow joint function,and increase the recovery degree of radial nerve injury,which is worthy of clinical promotion.
