摘  要:目的探讨3.0T高分辨磁共振成像(HR-MRI)T2WI在早中期宫颈癌术前分期中的应用价值。方法 31例早中期宫颈癌(FIGO分期IA^IIB)患者行常规T1WI(矢状位及斜轴位的非抑脂HR-MRI T2WI)检查,观察并对比MRI术前分期与术后病理分期结果的一致性。结果 31例早中期宫颈癌患者的MRI显示率为96.8%。以术后病理结果为金标准,MRI对早中期宫颈癌分期的诊断符合率为90.3%。随着MRI分期的增加,术后病理分期呈递增趋势(rs=0.950,P=0.000),宫颈癌患者术前MRI分期与术后病理分期的诊断一致性好(Kappa=0.852,P=0.000)。结论 HR-MRI能较准确地用于宫颈癌术前分期判断,有助于早期治疗方案的制定。Objective To investigate the accuracy of high-resolution magnetic resonance imaging(HR-MRI) for preoperative staging in the cervical cancer of the early-middle stage with Federation International of Gynecology and Obstetrics(FIGO)stage IA-IIB using T2 weighted sequence. Methods 31 patents with cervical cancer in the early-middle stage(FIGO stage IA-IIB)underwent 3.0 T MRI with routine T1 and sagittal plane with T2 WI and oblique-axial HR-MRI T2 WI(not fat suppression)were recruited. The consistency was compared and analyzed between the preoperative MRI staging and postoperative pathologic staging. Results 96.8% of thirty-one patents with cervical cancer in the early-middle stage were detected. The overall accuracy of HR-MRI in the diagnosis of preoperative staging of thirty-one patents with cervical cancer in the early-middle stage was90.3%. With the increase of MRI staging,postoperative pathologic staging was significantly increased. The good consistency was demonstrated for the MRI staging and postoperative pathologic staging(rs=0.950,P=0.000;Kappa=0.852〉0.75,P=0.000). Conclusion HR-MRI is an accurate technique for evaluating the preoperative staging of cervical cancer. It is helpful in the formulation of treatment.
