孕酮、雌二醇及人绒毛促性腺激素在稽留流产诊断中的临床价值

摘　　要：选择2015年1月—2015年10月我院门诊随诊孕龄为5~6周的稽留流产、先兆流产、正常妊娠患者各50例,动态比较3组孕妇血清中的孕酮、雌二醇、人绒毛促性腺激素(HCG)临床检测结果。结果显示:稽留流产组孕酮、雌二醇、HCG水平显著低于正常妊娠组及先兆流产组(P<0.05),48小时后稽留流产组的HCG倍增明显低于正常妊娠组和先兆流产组,数据有统计学意义(P<0.05),至1周后其HCG变化不大,同时稽留流产组孕酮和雌二醇水平随着孕期增长逐渐降低,与正常妊娠组与先兆流产组孕妇的差距逐渐增加。临床检测结果表明:动态联合监测血清孕酮、雌二醇及HCG的变化能在妊娠早期及时地对妊娠结局作出准确的预测,提高早期稽留流产的检出率,以便及早发现不良妊娠,指导临床治疗,避免盲目保胎。From January 2015 to October 2015 in our hospital outpatient follow-up at gestational age of 5 to 6 weeks of missed abortion,threatened abortion and normal pregnancy were 50 cases,compared 3 groups of pregnant women serum progesterone and estradiol, human chorionic gonadotropin（ HCG） clinical test results. The missed abortion group,estradiol,progesterone and HCG levels were significantly lower than those in normal pregnancy group and threatened abortiongroup（ P〈0. 05）,missed abortion group HCG doubled after 48 hours was significantly lower than that in normal pregnancy group and threatened abortion group,and was statistically significant（ P〈0. 05）,to 1 weeks after the HCG changed little,and missed abortion group of estradiol and progesterone levels during pregnancy with growth gradually decreased,and pregnant women of normal pregnancy and threatened abortion group gap is gradually increased. The dynamic changes of joint monitoring of serum progesterone（ P） and estradiol and human chorionic gonadotropin（ HCG） can make an accurate prediction of pregnancy outcome in early pregnancy timely,and improve the early detection rate of missed abortion,which can be used for early detection of adverse pregnancy,and guide clinical treatment and avoid blind miscarriage.
