摘 要:目的:测定临床分离的念珠菌对抗真菌药物那他霉素(natamycin,NAT)的敏感性,并研究NAT分别和氟康唑(fluconazole,FLU)、伊曲康唑(itraconazole,ITC)联合抗念珠菌效应。方法:参考美国临床实验室标准化研究所(Clinical Laboratory Standard Institute,CLSI)M27-A3方案测定NAT、FLU和ITC对40株念珠菌的最低抑菌浓度(minimal inhibitory concentration,MIC),同时以棋盘法测定NTA分别和FLU或ITC联合抗真菌作用。结果:微量液基稀释法测得NAT、FLU和ITC对白念珠菌(15株)MIC范围分别为:2.0~4.0、2~64μg/mL和2~16μg/mL;对克柔念珠菌(5株)MIC范围分别为:2.0~4.0、16~64μg/mL和0.5~2μg/mL;对光滑珠菌(10株)MIC范围分别为:1.0~4.0、4~32μg/mL和1~4μg/mL;对热带念珠菌(10株)MIC范围分别为:2.0、1~4μg/mL和0.5~2μg/mL。NAT与FLU在体外联合应用时,对67%(10/15)的白念珠菌、40%(4/10)的光滑念珠菌和对50%(5/10)的热带念珠菌变现为协同作用;NAT与ITC在联合应用时,对33%(5/15)的白念珠菌、20%(1/5)的克柔念珠菌和60%(6/10)的光滑念珠菌和对40%(4/10)的热带念珠菌变现为协同作用;NAT与FLU或ITC在体外联合应用时,对所有菌株均无拮抗作用。结论:念珠菌在体外对NAT敏感,当NAT与FLU或ITC联合时,对部分念珠菌有联合效应,无拮抗效应。Objectives:To investigate the in vitro antifungal susceptibility of Candida spp.isolated for clinic to Natamycin(NTA),as well as the combination with Fluconazole(FLU)and Itraconazole(ITC).Methods:The minimal inhibitory concentrations(MICs)of NTA,FLU,and ITC to 40 strains of Candida spp.were assayed by using CLSI M27-A3 microdilution method.Besides,to further detect the combination regimen,we tested the combination of NTA and FLU or ITC against Candida spp..Results:The MIC ranges of NTA,FLU and ITC against Candida albicanswere2.0~4.0μg/mL,2~64μg/mL and 2~16μg/mL;While the MIC ranges of against Candida krusei2.0~4.0μg/mL,16~64μg/mL and 0.5~2μg/mL,MIC ranges of against Candida glabratawere1.0-4.0μg/mL,4~32μg/mL and 1~4μg/mL,and MIC ranges of against Candida tropicalis were2.0μg/mL,1~4μg/mL and 0.5~2μg/mL.The combination of NTA with FLU against 67%(10/15)of C.albicans,40%(4/10)of C.glabrata,and 50%(5/10)of C.tropicalis showed synergism.By the way,the combination of NTA with ITC against 33%(5/15)of C.albicans,20%(1/5)of C.krusei,60%(6/10)of C.glabrata,and 40%(5/10)of C.tropicalis showed synergism.There were no antagonism were found in the combinations.Conclusion:Candida spp.is susceptible to NTA,and the combinations of NTA with FLU or ITC showed synergism against some of the Candida spp.but no antagonism was found.
