Association between fasting plasma glucose level and osteoporosis among the elderly aged over 60 years:a population-based cross-sectional study in rural China

摘　　要：目的:骨质疏松症(OP)是老年人中最常见的代谢性骨病,但其与空腹血糖(FPG)的关系尚不清楚。本研究旨在探讨中国北方农村低收入老年人中骨质疏松与空腹血糖水平的关系。方法:本研究招募1877例60岁以上老年人,测量骨矿物质密度、FPG和脂质水平。结果:空腹血糖每增加1mmol/L,骨质疏松患病率增加25%(P<0.01)。在空腹血糖正常组和糖耐量异常(IGT)组中,女性是骨质疏松的独立危险因素,年龄是空腹血糖正常组和糖尿病组重要的危险因素,仅在80岁以上糖耐量受损组中观察到年龄对骨质疏松的显著影响(OR=5.94;95%CI:1.43~24.73,P<0.05)。空腹血糖正常组的甘油三酯每增加1mmol/L,骨质疏松患病率下降33%,而IGT组BMI每增加1kg/m^2,骨质疏松患病率下降12%。结论:中国农村地区骨质疏松的患病率处于较居高水平,控制血糖等相关危险因素对降低该人群骨质疏松的负担至关重要。Objective：Osteoporosis （OP） is the most common metabolic bone disorder in the elderly.However,the relationship between OP and fasting plasma glucose （FPG） is still unknown.In this study,we aimed to explore the association between OP and FPG levels among a low-income elderly population in rural China.Methods：We recruited 1877 participants aged ≥60 years in northern rural China.Bone mineral density,FPG and lipid levels were measured for the aged person.Results：With an increase of 1 mmol/L of fasting blood sugar,the prevalence of OP increased by 25%.Gender was an independent risk factor for OP in normal FPG group and impaired glucose tolerance （IGT） group.Age was an important risk factor in normal FPG group and IGT group,but not in diabetic group.The prominent effect of age to OP in IGT group was observed only among the person over 80 years （odds ratio = 5.94,95% confidence interval：1.43 ~ 24.73,P 0.05）.The triglycerides level in normal FPG group increased by each 1 mmol/L,the prevalence of OP decreased by 33%.In IGT group,the prevalence of OP decreased by 12% with an increase of 1 kg/m^2 per BMI.Conclusions：The prevalence of OP in rural China was at a higher level.Controlling blood sugar and other related risk factors are critical to reducing the burden of OP in this population.
