读《金匮要略方论》论治肝衰竭

摘　　要：肝衰竭属于中医学黄疸病中“急黄…‘瘟黄”范畴，在治疗上应遵循黄疸的基本治疗原则。《金匮要略方论》系统阐述了黄疸病的理、法、方、药，对后世影响颇深。本文从《金匮要略方论》出发，探讨肝衰竭的论治，认为肝衰竭具有“湿、热、瘀”黄疸病的病理基础，但疾病过程中表现出来的热象更为明显，后期“虚”象明显；同时总结出肝衰竭治疗上应遵守黄疸病清热利湿的基本治疗方法，重用清热解毒，截断病势，活血化瘀当贯穿始终，同时调和脾胃，温补脾。肾之阳，注意重用茵陈、先煎茵陈。Liver failure belongs to the category of jaundice and yellow plague in the traditional Chinese medicine. It has something in common with the etiology and pathogenesis of jaundice. In Synopsis of Prescriptions of the Golden Chamber, the theories, methods, prescriptions and drugs for jaundice are systemically explained, and the related therapeutic principles and methods are proposed, that have profound influence on the treatment of jaundice for later generations. From the point of view upon the treatment of jaundice in Synopsis of Prescriptions of the Golden Chamber, the treatment of liver failure is discussed in this article. The author thinks that the ＆quot;dampness, heat and blood stasis＆quot; are the pathological bases of jaundice, but the heat phenomenon is more obvious in the course of the disease, and at later stage＆quot;deficiency＆quot; is significant; the basic principle of the treatment of liver failure should first comply with clearing away heat and promoting diuresis, using the drugs heavily for heat-clearing and detoxifying to block the disease as the main line especially at the beginning stage, drugs for activating blood circulation and removing blood stasis are applied from the beginning to the end of the treatment, and in the mean time, regulation of spleen and stomach and tonification of spleen and kidney should be paid attention to especially at the later stage. The application and skill of making Yinchenhao decoction for treatment of jaundice is also important.
