摘　　要：目的探讨急性心肌梗死（AMI）急诊PCI术后应用左卡尼汀，对血清心型脂肪酸结合蛋白（H-FABP）及近期预后的影响。方法选择AMI行急诊PCI患者102例，随机分为治疗组54例和对照组48例，实验组在急诊PCI中加用左卡尼汀3．0g＋0．9％生理盐水250mL静脉滴注，以后每日1次，连续7d以上。对照组行急诊PCI治疗及常规药物治疗。比较两组血浆H-FABP、CK-MB、cTnI水平和住院期间LVEF值，随访90d后六分钟步行试验（6MWT）距离。结果实验组术后血浆H-FABP、CK-MB、cTnI水平较对照组明显降低，LVEF高于对照组（P〈0．05），随访90d后实验组6MWT距离明显高于对照组（P〈0．05）。结论AMI急诊PCI患者补充左卡尼汀对缺血心肌细胞有保护作用，可明显改善患者的心功能。Objective To explore the effect of L - carnitine on content of heart fatty acid binding protein （ H - FABP） and the influence on the recent prognosis in acute myocardial infarction （AMI） patients with emergent PCI. Methods A total of 102 AMI patients with PCI were randomly divided into two groups, one was routine treatment group ： treated with routine therapy ; another was L - carnitine treatment group： treated with L - carnitine every day on the basis of above routine treatment. H - FABP, CK - MB, cTnI concentrations, LVEF and 6 - minute walk test （6MWT） were compared between the two groups. Results The difference of plasma heart - type fatty acid - binding proteins （H- FABP）, creatine kinase isoenzyme （CK- MB）, cardiac troponin I（cTnI） level and left ventricle ejection fraction （ LVEF ） was statistically significant （ P 〈 0. 05 ） in two groups. Walking distance evaluated by 6MWT was significantly higher in L - carnitine treatment group after three months of follow- up. Conclusion L- carnitine shows protective effect against the ischemia injury in cardiomyocytes and improves cardiac function for the emergency PCI patients with ST- elevation myocardial infarction.
