自拟益阴活血汤配合针刺对2型糖尿病周围神经病变神经电生理的影响

摘　　要：目的:观察自拟益阴活血汤配合针刺对2型糖尿病周围神经病变神经传导速度的影响。方法:将180例2型糖尿病周围神经病变患者随机分为对照1组、对照2组、对照3组、治疗组。4组均予基础降糖治疗,对照1组给予甲钴胺治疗,对照2组给予自拟益阴活血汤治疗,对照3组给予针刺治疗,治疗组给予自拟益阴活血汤联合针刺治疗。4组均以4周为1个疗程,治疗1个疗程后比较治疗前后双侧正中神经及腓总神经运动传导速度及感觉传导速度的差异,并评价综合疗效。结果:治疗组总有效率为82.22%,明显优于其他3组,组间比较,差异有统计学意义(P<0.05);4组双侧正中神经、腓总神经运动传导速度及感觉传导速度治疗前后组内比较及治疗后组间比较,差异均有统计学意义(P<0.05)。结论:自拟益阴活血汤配合针刺治疗对2型糖尿病周围神经病变运动传导速度及感觉传导速度均有明显改善,值得临床推广应用。Objective ： To inz＇cstigate the effect of self - made yin - nourishing and blood circulation - activating decoction combined with acupuncture on nerve conduction velocity in patients with type 2 diabetic peripheral neu- ropathy. Methods：A total of 180 patients with type 2 diabetic peripheral neuropathy were randomly divided into con- trol groups 1,2, and 3 and treatment group. All patients were given basic hypoglycemic therapy. The patients in con- trol group 1 were given mecobalamin, those in control group 2 were given self - made yin - nourishing and blood cir- culation -activating decoction, those in control group 3 were given acupuncture, and those in treatment group were given self- made yin -nourishing and blood circulation -activating decoction combined with acupuncture. The course of treatment was 4 weeks for all groups, and the four groups were compared in terms of motor conduction ve- locity and sensory conduction velocity in both median nerves and both common peroneal nerves and clinical outcome. Resuhs ：The treatment group had a significantly higher overall response rate （82.22%）than the other three groups （ P 〈 0. 05 ）. All groups had significant changes in motor conduction velocity and sensory conduction velocity in both median nerves and both common peroneal nerves after treatment, and there were significant differences in these indices between the four groups after treatment （ P 〈 0.05 ）. Conclusion ： In patients with type 2 diabetic pe- ripheral neuropathy, self - made yin - nourishing and blood circulation - activating decoction combined with acu- puncture can significantly improve motor conduction velocity and sensory conduction velocity and thus holds promise for clinical application.
