经尿道等离子电切术在前列腺增生中的应用效果及对尿动力学参数的影响

摘　　要：目的 研究经尿道等离子电切术在前列腺增生中的应用效果及对尿动力学参数的影响.方法 选取2013年2月至2016年1月到本院就诊的前列腺增生患者200例,按照随机抽签的方法将其分为观察组和对照组,每组100例,观察组运用经尿道等离子电切术进行治疗,而对照组则使用经尿道前列腺电切术进行治疗,观察两组患者的手术时间、术后住院时间、并发症发生率、生活质量评分（QOL）、国际前列腺症状评分（IPSS）等疗效指标,以及术后5个月的残余尿量、初始尿意容积、最大尿流率等尿动力学参数.结果 治疗后,观察组的患者在手术时间、住院时间及并发症发生率均优于对照组（P＜0.05）,且观察组术后QOL明显低于对照组（P＜0.05）;术后5个月,两组的残余尿量、初始尿意容积、最大尿流率尿动力学参数较术前均有明显改善（P＜0.05）,且除初始尿意容积外,观察组术后5个月的其他尿动力学参数均明显好于对照组,两组比较差异均有统计学意义（P＜0.05）.结论 经尿道等离子电切术治疗前列腺增生疗效显著,安全、并发症少,可明显改善患者的尿动力学参数,值得临床推广应用.Objective To investigate transurethral plasmakinetic resection on benign prostatic hyperplasia and its influence on urodynamic parameters.Methods A total of 200 patients with benign prostatic hyperplasia in our hospital from February 2013 to January 2016 were randomly divided into two groups：observation group and control group,（100 cases in each group）.The observation group was treated with transurethral plasmakinetic resection,and the control group was treated with transurethral resection of prostate.Operative time,postoperative stay,complication rates,quality of life （QOL）,International Prostate Symptom Score （IPSS） and residual urine volume at 5 months after operation,initial urine volume and maximum urinary flow rate were observed.Results After treatment,the operative time,hospital stay and complication rate of the observation group were better than those of the control group （P ＜0.05）,and the QOL of the observation group was significantly lower than that of the control group （P ＜ 0.05）.In addition to the initial urinary volume,the other urine in the observation group were significantly higher than that before the operation （P ＜ 0.05）,and the urine volume of the initial urine volume,the initial urine volume and the maximum urinary flow rate were significantly improved,and the dynamic parameters at 5 months after operationwere significantly better than those in the control group （P ＜ 0.05）.Conclusions Transurethral plasmakinetic prostatectomy for the treatment of benign prostatic hyperplasia has advantages of significant efficacy,safety,fewer complications,it can significantly improve the patient＇s urodynamic parameters,worthy of clinical application.
