25羟维生素D的水平变化与结缔组织病相关肺间质病变的相关性分析

摘　　要：目的探究25羟维生素D(25OH-D)的水平与结缔组织病相关肺间质病变(CTD-ILD)的相关性。方法选取2016年1月至12月我院收治的68例CTD-ILD患者,设为观察组,选取同期70名健康体检者作为对照组,采用酶联免疫法检测2组受检者血清25OH-D水平并进行对比,同时对25OH-D水平与体质量指数(BMI)、红细胞沉降率(ESR)、C反应蛋白(CRP)、类风湿因子(RF)、抗核抗体(ANA)、补体C3、C4、一氧化碳弥散量占比(DLCOSB)、1s率(FEV1/FVC)的相关性进行统计分析。结果观察组25OH-D平均为(25±4)ng/mL,显著低于对照组的(46±11)ng/mL,且观察组25OH-D缺乏率达到93%(63/68),显著高于对照组的6%(4/70),差异均有统计学意义(P<0.05);不同类型结缔组织病患者之间的25OH-D水平对比差异无统计学意义(P>0.05);CTD-ILD患者血清25OH-D水平与BMI、ESR、FEV1/FVC均呈负相关(P<0.05),与DLCOSB水平呈正相关(P<0.05),与其他因素无明显相关(P>0.05)。结论 CTD-ILD患者的血清25OH-D水平明显低于健康人群,且与患者BMI、ESR及肺功能有关,应当引起重视。Objective To investigate the correlation between the level of 25-hydroxy-vitamin D （25OH-D） and connective tissue disease-related pulmonary interstitial lesions （CTD-ILD） .Methods Sixty-eight patients with CTD-ILD were enrolled in this study .The subjects were selected as the observation group .Seventy healthy subjects were selected as the control group .Two groups of subjects were tested by enzyme-linked immunosorbent assay （ELISA） .Body mass indes （BMI） ,erythrocyte sedimentation rate （ESR） ,C-reactive protein （CRP） , rheumatoid factor （RF） ,antinuclear antibody （ANA） ,complement and the levels of 25OH-D were compared . And 25OH-D C3 ,C4 ,carbon monoxide diffusion ratio （DLCOSB） and one second rate （FEV1/FVC） were statis-tically analyzed .Results The average level of 25OH-D in the observation group was （25 ＆#177; 4）ng/mL ,which was significantly lower than that in the control group （46 ＆#177; 11）ng/mL （P〈0.05） ,and the 25OH-D deficiency rate in the observation group was 93% （P〈0.05） ,which was significantly higher than that in the control group （6% ） （P〈0.05） .There was no significant difference in the level of 25OH-D between the patients with different types of connective tissue disease （P〉0.05） .The level of serum 25OH-D in CTD-ILD patients was negaitively correla-ted with BMI ,ESR ,FEV1/FVC （P〈0.05） ,and positively correlated with DLCOSB （P〈0.05） ,and there was no significant correlation with other factors （P〉0.05 ） .Conclusion The level of serum 25OH-D in CTD-ILD pa-tients is significantly lower than that in normal subjects ,and it is related to the BMI ,ESR and pulmonary func-tion .
