夹脊穴电刺激结合艾灸法对肌萎缩侧索硬化患者肌电图的干预效应研究

摘　　要：目的:通过夹脊穴电刺激结合艾灸法治疗肌萎缩侧索硬化,观察其对患者肌电图的影响,旨在探索治疗肌萎缩侧索硬化的有效途径。方法:将40例肌萎缩侧索硬化患者随机分为治疗组和对照组,每组20例,治疗组选用华佗夹脊穴(C5~6、C6~7、C7~T1、L3~4、L4~5、L5~S1)电针疏波刺激,30 min/次,同时结合关元、足三里穴艾条灸法,10 min/穴,1次/日;对照组只进行艾条灸法治疗。2周1个疗程,休息1天,进行下一疗程,共4个疗程。治疗前后给予肌电图检测,并对检测结果进行统计、评价。结果:治疗组和对照组治疗前后比较,大部分组别失神经电位减少,波幅增宽增高例数减少,单纯相出现例数减少,具有显著性差异(P<0.05,P<0.01);治疗后两组肌电图比较,治疗组较对照组大部分失神经电位减少,波幅增宽增高例数减少,单纯相出现例数减少,具有显著性差异(P<0.05,P<0.01)。结论:夹脊穴电刺激配合艾灸治疗ALS可以改善球、颈、胸、腰骶肌肉的肌电图参数,表明此方法治疗ALS行之有效,值得临床推广。Objective： To explore effective treatment for amyotrophic lateral sclerosis（ALS） by observing the electromyo- gram（EMG） of ALS patients treated by electrical stimulation on Jiaji points combined with moxibustion. The research was designed to find an effective method of treating ALS. Methods： 40 ALS patients were randomly divided into the treatment group and the control group. The treatment group was intervened by electro - acupuncture on Huatuo Jiaji points（C5-6, C6-7, C7 - Tl, L3-4, L4-5, and L5 - S1 ） with sparse wave, combined with moxibustion on CV4 and ST36. The control group was treated with moxibustion alone. Two Weeks was taken as one course, one day break be- tween each course, the total treatment was four courses. EMG was observed and analyzed before and after the treatment between the two groups. Results： After treatment, eases of denervation potential, widening and heightening amplitude, and simple phase were significantly reduced（ P 〈 0.05, P 〈 0.01 ） ; of which, the treatment group was superior to the control group（P 〈0.05, P 〈 0.01 ）. Conclusion： The therapy of electrical stimulation on Jiaji points combined with moxibustion can improve the EMG on muscles dominated by bulbus medullae, muscles of neck, chest and lumbosacral parts, which proves its efficiency and clinical worth.
