溃疡性结肠炎和克罗恩病的自主神经功能变化

摘　　要：目的探讨溃疡性结肠炎(UC)和克罗恩病(CD)的自主神经功能变化特点。方法选择2015年1月~2016年12月在本院治疗的100例UC患者、100例CD患者纳入研究,设定为UC组、CD组,选取100例同期健康体检者设定为正常组,各组间两两比较迷走副交感神经功能(卧立位心率变化值、瓦氏动作反应指数)和交感肾上腺能神经功能(卧立位血压差、握力试验血压差)。结果 UC组、CD组的卧立位心率变化值和握力试验血压差均显著性低于正常组(P<0.05),UC组、CD组的卧立位血压差明显高于正常组(P<0.05);轻度UC组的卧立位心率变化值、握力试验血压差均明显高于中度UC组(P<0.05),轻度UC组的卧立位血压差明显低于中度UC组(P<0.05);轻度CD组的卧立位心率变化值、Valsalva动作反应指数、握力试验血压差均显著性高于中度CD组(P<0.05),轻度CD组的卧立位血压差明显低于中度CD组(P<0.05)。结论 UC和CD患者存在自主神经功能紊乱症状,表现为迷走神经功能减弱、交感神经功能增强,且其紊乱程度与病情轻重呈正相关。Objective Objective To investigate the changes of autonomic nervous function in patients with ulcerative colitis （UC） and crohn disease（CD）. Methods The 100 cases of patients with UC and 100 cases of patients with CD admitted to hospital from January 2015 to December 2016 were selected as UC group and CD group, and 100 cases of healthy examinations people were selected as normal group. The vagus parasympathetic function （lying to standing heart rate, Valsalva maneuver index） and sympathetic nerve function（postural blood pressure change, sustained handgrip test） among groups were observed and compared. Results The lying to standing heart rate and sustained handgrip test in the UC group and CD group were significantly lower than the normal group （P 〈0. 05 ） ; the postural blood pressure change in the UC group and CD group were significantly higher than the normal group （P 〈 0. 05 ） ; the lying to standing heart rate and sustained handgrip test in the mild UC group were significantly higher than in moderate UC group（P 〈 0.05 ） ; the postural blood pressure change in the mild UC group were significantly lower than in moderate UC group（ P 〈 0. 05） ; the lying to standing heart rate, Valsalva maneuver index and sustained handgrip test in the mild CD group were significantly higher than in moderate CD group（ P 〈 0. 05 ） ; the postural blood pressure change in the mild CD group were significantly lower than in moderate CD group（P 〈 0. 05 ）. Conclusion UC and CD patients show autonomic dysfunction, with decreased vagus nerve function but elevated sympathetic nerve function. And the degree of autonomic dysfunction is correlated with the severity of disease.
