超声引导下术前导丝定位在乳腺微小占位病变的临床价值

摘　　要：目的超声引导下术前导丝定位在乳腺微小占位病变的临床价值。方法选取我院2016年1月至2017年4月28例健康体检诊断出有乳腺微小占位病变的患者,且经过触诊为阴性,术前在超声引导下进行导丝定位,对疾病类型、位置进行诊断,并通过病理学检查来验证结果准确性,探讨该技术在乳腺疾病的临床诊断价值。结果经超声引导下导丝定位技术确定,28例病灶处的最大肿瘤为0.82 cm×1.53 cm,最小为0.43 cm×0.55 cm。共有20例(71%)良性病变和8例(29%)恶性病变,诊断准确率为100%,出现了1例(4%)轻微不良反应;28例需要进行穿刺定位后乳腺占位切除者有5例早期乳腺癌,2例乳腺炎性结节,5例乳腺腺病,1例导管内乳头状瘤,12例乳腺纤维瘤和3例浸润癌。结论超声引导下术前导丝定位可以准确诊断微小病变,降低创伤,而且高诊断性和病灶位置锁定技术有利于在术中迅速找到病变,缩短手术时间,对乳腺癌的防治以及预后具有深远意义。Objective To evaluate the clinical value of preoperative guide wire localization in mammary gland lesions. Methods From January 2016 to April 2017, 28 cases of healthy physical examination to diagnose patients with small breast lesions, and after palpation was negative, preoperative ultrasound guided guided wire positioning, the type of disease, the location of the diagnosis, and through pathology to verify the accuracy of the results, to explore the clinical diagnosis of breast disease in the value of the disease. Results The maximal tumor of the lesion was 0.82 cm×1.53 cm and the minimum was 0.43 cm×0.55 cm. A total of 20 cases （71.42%） of benign lesions and 8 cases （28.57%） of malignant lesions, the diagnostic accuracy rate of 100%, there was one case （3.57%） mild adverse reactions; 28 cases need to be puncture positioning after breast resection with 5 early breast cancer, 2 cases of breast inflammatory nodules, 5 cases of breast adenosis, 1 cases of intraductal breast cancer, 12 cases of breast fibroma and 3 cases of invasive carcinoma. Conclusion Ultrasound-guided preoperative guideline localization can accurately diagnose minute lesions and reduce trauma, and high diagnostic and lesion location locking technique is helpful for rapid detection of lesions during surgery and shorten the operation time, and the prevention and treatment of breast cancer and prognosis far-reaching significance.
