摘 要:目的观察踝臂指数(ankle brachial index,ABI)、脉搏波传导速度(pulse wave velocity,PWV)、颈动脉内中膜厚度(carotid artery intima-media thickness,cIMT)与冠状动脉狭窄严重程度的相关性,以评估ABI、PWV、IMT对冠状动脉狭窄严重程度的预测价值。方法选取2010年10月至2014年11月在海口市人民医院行冠状动脉造影术的患者301例作为研究对象。所有研究对象在造影前均行常规生化及ABI、PWV、cIMT检查。根据冠状动脉造影结果,分为冠心病组和非冠心病组;冠心病组再分为冠状动脉单支病变亚组、双支和多支病变亚组,分析ABI、PWV、cIMT与冠状动脉狭窄程度的相关性。结果非冠心病组ABI、PWV、cIMT与冠状动脉单支病变亚组、双支和多支病变亚组比较,差异均有统计学意义(P<0.01)。Spearman等级相关分析结果表明PWV、cIMT均与冠状动脉狭窄程度呈正相关(r=0.549,P<0.05;r=0.781,P<0.05);ABI与冠状动脉狭窄程度呈负相关(r=-0.555,P<0.05)。结论 ABI、PWV、cIMT与冠状动脉病变狭窄程度具有明显相关性。联合应用ABI、PWV和cIMT可用于评估冠状动脉粥样硬化性心脏病(冠心病)的严重程度。Objectives To observe the correlation among severity of coronary artery stenosis(CAS)and ankle brachial index(ABI),pulse wave velocity(PWV)and carotid artery intima-media thickness(cIMT)respectively;and to eval-uate the predictive value of ABI ,PWV and cIMT on severity of CAS. Methods We selected 301 patients underwent coronary angiography(CAG)from October 2010 to November 2014 in Haikou People&#39;s Hospital as study subjects. All the patients were measured routine biochemistry and ABI,PWV,cIMT before CAG. According to the results of CAG, patients were divided into non coronary heart disease group and coronary heart disease group. Coronary heart disease group were further divided into single vessel lesion subgroup,double vessel lesion subgroup and muti-vessel lesion sub-group. Correlations of ABI,PWV,cIMT between severity of coronary artery stenosis were analyzed. Results ABI, PWV,cIMT in non coronary heart disease group had significantly differences comparing with those in single vessel le-sion subgroup,double vessel lesion subgroup and muti-vessel lesion subgroup(P〈0.01). Spearmanrank correlation anal-ysis indicated that PWV,cIMT positively correlated with severity of coronary artery stenosis(r=0.549,P〈0.05;r=0.781, P〈0.05);ABI negatively correlated with severity of coronary artery stenosis(r=-0.555,P〈0.05). Conclusions ABI, PWV and cIMT have obvious correlations with stenosis of coronary artery disease. Combined application of ABI ,PWV and cIMT can be used for evaluating the severity of coronary atherosclerotic heart.
