摘 要:目的分析原发性高血压(高血压)患者血清谷氨酰转肽酶浓度与左心室肥厚的关系及临床意义。方法2013年4月至2015年12月从鹤山市人民医院选取资料完整的新发高血压患者200例,根据左心室质量指数分为左心室肥厚组及左心室正常组。比较两组的一般临床资料、生化指标和超声心动图参数,并采用多因素回归分析左心室肥厚的相关因素。结果在高血压患者当中,左心室肥厚组血清谷氨酰转肽酶浓度明显低于左心室正常组,差异有统计学意义(P>0.05)。Spearman相关分析表明血清谷氨酰转肽酶浓度与左心室质量指数负相关(r=-0.457,P=0.002);多变量逐步Logistic回归分析结果显示血清谷氨酰转肽酶浓度(OR=0.534,95%CI:0.353~0.868;P=0.022)是左心室肥厚的保护因素。结论高血压患者血清谷氨酰转肽酶浓度与左心质量指数呈负相关,监测血清肝酶变化可能对早期发现左心室重构有一定意义。Objectives To analyze the relationship and clinical meaning of serum concentration of serumglutamyl trans-peptidase(GGT)with left ventricular hypertrophy in patients with essential hypertension. Methods All the newly di-agnosed 200 essential hypertension patients were from April 2013 to December 2015 in Heshan People's Hospital. They were divided into none left ventricular hypertrophy and left ventricular hypertrophy group based on left ventricular mass index. Clinical dates ,biochemical indicators and ultrasonic cardiogram parameters between the two groups were com-pared. Multivariate Logistic regression analysis was used for finding the related factors for left ventricular hypertrophy. Results Serum concentration of GGT was significantly lower in left ventricular hypertrophy group than in none left ven-tricular hypertrophy group(P〈0.05). Spearman correlation showed that serum concentration of GGT negatively associated with left ventricular mass index(r=-0.457,P=0.002). Multiple Logistic regression analysis showed that GGT(OR=0.534, 95%CI:0.353-0.868;P=0.022)was a protective factor for left ventricular hypertrophy. Conclusions Serum concentra-tion of GGT negatively correlates with left ventricular mass index among essential hypertension patients ,and monitoring the changes of serum liver enzyme may has significant effect on the early detection of left ventricular hypertrophy.
