高血压患者左心室室壁厚度与左心室舒张功能的相关性

摘　　要：目的探讨高血压(含原发性及继发性)患者左心室室壁厚度与左心室舒张功能之间的联系。方法随机抽取惠州市中心医院2015年1月至2015年8月间收治的符合高血压左心室肥厚入选标准的患者157例作为实验组,以及同期非高血压患者600例作为对照组。采用飞利浦IE33彩色多普勒超声诊断系统检测计算两组患者的室间隔厚度(IVST)、左心室后壁厚度(LVPWT)、舒张早期和舒张晚期二尖瓣口最大血流速度之比(E/A)以及左心室射血分数(LVEF)等指标。对两组的检测结果进行统计学处理分析。结果实验组的室间隔厚度和左心室后壁厚度显著性高于对照组,差异有统计学意义[(1.39±0.19)cm vs.(0.93±0.11)cm,P<0.01;(1.34±0.29)cm vs.(0.95±0.13)cm,P<0.05];并且,高血压左心室肥厚患者的A峰值增高、E峰下降,使E/A值>1,并显著性低于对照组患者,差异有统计学意(0.9±0.2 vs.1.3±0.3,P<0.01)。结论高血压伴心室肥厚的患者的心室舒张功能可能同时存在损伤。Objectives To investigate the relationship between left ventricular wall thickness and diastolic function in hypertensive patients（including primary and secondary hypertension）. Methods A total of 157 hypertensive patients with increased left ventricular wall thickness were randomly enrolled in Huizhou Central Hospital from January to August of 2015 and were treated as experimental group ,another 600 normotensive subjects were recognized as control group. Echocardiography （Philips IE33） was used to evaluate the inter-ventricular septal thickness （IVST）,left ventricular posterior wall thickness （LVPWT）,ratio of the fastest and slowest velocities of trans-mitral valve during early and late diastolic period （E/A） and left ventricular ejection fraction （LVEF）. Comparisons of these two groups were performed by statistical analysis. Results Compared to control group ,IVST and LVPWT were both significantly higher in experimental group[（1.39±0.19）cm vs.（0.93±0.11）cm,P〈0.01;（1.34±0.29）cm vs.（0.95±0.13）cm, P〈0.05];and the E wave increased while the A wave reduced resulting in E/A ratio〉1 in hypertensive patients which was significantly lower than that in control group（0.9±0.2 vs. 1.3±0.3,P〈0.01）. Conclusions Hypertensive patients with increased left ventricular wall thickness may be suffering diastolic dysfunction.
