中国汉族人群HLA-B*5801等位基因与别嘌醇致重症药疹的相关性Meta分析

摘　　要：目的探讨中国汉族人群人白细胞抗原-B*5801(HLA-B*5801)等位基因与别嘌醇致重症药疹(SCAR)的关系。方法通过Review Manager5.3软件对筛选出的9项国内外有关中国汉族人群HLAB*5801等位基因与别嘌醇致SCAR的文献进行综合评价,比较别嘌醇SCAR组、别嘌醇耐受组以及健康对照组组间HLA-B*5801等位基因比率,并进行异质性检验、合并效应量分析;应用Stata12.0进行Begg检验和Egger检验对潜在的发表偏倚进行评估。结果别嘌醇SCAR组与健康对照组间存在异质性(P=0.02),合并效应量[OR(95%CI)=78.6(27.21~227.08)],综合检验(P<0.001);别嘌醇SCAR组与别嘌醇耐受组间无异质性(P=0.17),合并效应量[OR(95%CI)=148.03(73.03~300.07)],综合检验(P<0.001);别嘌醇耐受组与健康对照组间无异质性(P=1.00),合并效应量[OR(95%CI)=0.68(0.48~0.96)],综合检验(P=0.03)。结论中国汉族人群HLA-B*5801等位基因与别嘌醇致SCAR具有相关性,患者服用别嘌醇前应进行HLA-B*5801等位基因检查,有助于减少别嘌醇所致SCAR的发生。Objective To explore the relationship between HLA-B＊5801 allele,allopurinol and its related severe cutaneous adverse reactions in Chinese Han population.Methods Comprehensive evaluation using Review Manager 5.3 software to 9 domestic screened abroad Chinese Han population HLA-B＊5801 allele and allopurinol induced SCAR literature, Comparison of allopurinol resulted in SCAR group, allopurinol tolerance group and control group group HLA-B＊5801 allele ratio, heterogeneity test, analysis of the combined effect of the amount of; Stata12.0 performed Begg test and Egger test to evaluate potential publication bias.Results There is heterogeneity between allopurinol SCAR group and healthy control group（P=0.02,）,effect size[OR（95%CI）=78.6（27.21-227.08）],the for overall effect（P〈0.001）; No heterogeneity between allopurinol SCAR group and allopurinol tolerance group（P=0.17）,effect size[OR（95%CI）=148.03（73.03-300.07）],the for overall effect（P〈0.001）; No heterogeneity between allopurinol tolerance group and healthy control group（P=1.00）,effect size[OR（95%CI）=0.68（0.48-0.96）],the for overall effect（P=0.03）.Conclusion Correlation between China Han population allele HLA-B＊5801 and SCAR, patients taking allopurinol should be carried out before the HLA-B＊5801 allele, helps to reduce the incidence of SCAR caused by allopurinol.
